• 1st client= 120/80 mmhg
• 2nd client= 110/70 mmhg
• 3rd client = 130/90 mmhg
• 4th client = 150/90 mmhg
• 5th client = 110/90 mmhg
• 6th client = 100/80 mmhg
 7th client = 140/80 mmhg
 8th client = 122/70 mmhg
2 clients were elevated blood pressure, referred to city
health office for further management.

Assist client for clients were visited
consultation  1st client= hypertensive client= increase fluid
intake, encouraged to eat nutritious foods,
regular exercise and referred to city health for
further management.
 2nd client= headache= blood pressure taken,
health teaching done, advise to increase fluid
intake at least 2 liters per day, referred o city
health office for further management
 3rd client= joint pain= advised client to avoid
eating foods that contained beans, encouraged
to eat nutritious foods and referred client to city
health for further management.

consultation  1st client= skin rashes= encourage client to eat
nutritious foods, proper hygiene emphasized
and referred to city health for further
management.
 2nd client= dizziness= blood pressure taken,
health teaching done, emphasized the
importance of taking maintenance medication,
advise to increase fluid intake at least 2 liters
per day, referred to city health office for further
management
 3rd client= stiff neck = health teaching done and
referred client to city health for further
management.
 4th client = animal bite= health teaching done,
emphasized the importance of wound washing
technique and referred to city health office for
further management
