In compliance with the Guidelines issued by the National Task
Force (NTF) for Coronavirus Disease-2019 (COVID-19), Order
Number: 2020 - 02 dated May 13, 2020 with subject:
Operational Guidelines on the Management of Locally
Stranded Individuals (LSIs) signed by Delfin N. Lorenzana,
Chief of National Task Force Against Covid -19, Baguio City will
implement the following guidelines and procedure for Locally
Stranded Individuals.
RETURNING BAGUIO RESIDENTS (RBRs) FAQs
A. Who are RBRs? RBRs are Baguio Residents previously
stranded in other provinces during the ECQ and who are
now returning home. These include displaced workers,
students, residents who are coming from tourist or leisure
trips, and other stranded individuals.
B. What is the procedure to be followed by RBRs who

want to come home to Baguio City:
1. Secure a Medical Certificate from the City/Provincial
Health Office of the Place of Origin based on ANY of
the following conditions (as may be applicable):
a. The applicant is not a probable, suspect or
COVID-19 confirmed case;
b. He/She has completed the 14-day
quarantine period according to the
quarantine guidelines of the DOH;
c. The applicant is a COVID-19 confirmed
case who has tested negative through RT-
PCR twice;
2. Online registration of RBRs by filling–out a Health
Declaration Form (HDF) at http://hdf.baguio.gov.ph/.

Registration is currently on-going. Registration on
said site does NOT AUTOMATICALLY ALLOW the
RBR to enter the City.
3. RBR should upload a scanned copy of his/her
Medical Certificate and ANY ONE of the following
Proof of Residence (Driver’s License, Voter’s ID,
TIN ID, UMID ID, OR other valid IDs indicating
Baguio City as their Residence Address). If the RBR
has an existing Travel Authority, he/she must upload
the same.
4. The City Disaster and Risk Reduction Management
Office (CDRRMO) will validate the correctness of
the information and authenticity of the documents
uploaded by the RBR applicant.
5. If the RBR’s application is approved, CDRRMO will
schedule the RBR’s date of travel to the City and
inform the applicant. RBRs travelling together with
family members will not be separated. All members
of a family regardless of age should register.
6. CDRRMO will provide a list of the names and
schedules of approved RBR applicants to the
Tactical Operations Center (TOC) of the Baguio City
Police Office (BCPO) and the City’s Triage Units.
7. RBR should go to the nearest Philippine National
Police (PNP) office of Point of Origin and apply for a
Travel Authority signed and approved by the PNP
Regional Director. The RBR should inform the PNP
of his/her date of travel as scheduled by CDRRMO
and submit the Medical Certificate as stated in No.1
above.
8. RBR should present the Travel Authority issued by
the PNP upon arrival at Checkpoints.

9. Upon arrival to Baguio City, RBR will undergo Triage
and will either be (i) quarantined in a holding facility,
OR (ii) home quarantined and monitored by the
concerned Barangay Health Emergency Response
Teams (BHERTS). The Baguio City Police Office
(BCPO) will escort RBRs going to Triage to ensure
compliance.
C. Why is there a need for online registration at
http://hdf.baguio.gov.ph/?
Online Registration is necessary for the efficient processing

of RBR applications. This will also allow the Returning
Baguio Residents Task Force (RBRT) to know the volume
of RBRs that will be entering the City. By knowing this
information beforehand, necessary protocols will be
effectively implemented and managed and the City’s health
facilities will not be overwhelmed.
D. What is Point of Origin?
It is the City or Province where the RBR is stranded or

coming from.
E. Is a Medical Certificate issued by the City/Provincial

Health Office from Point of Origin mandatory?
Yes. Without the required Medical Certificate, the PNP will

not issue a Travel Authority.

F. Will RBRs who submit Health/Medical Certificates be
exempt from undergoing Triage.
No. The submission of a Health Certificate will not exempt

RBRs to undergo Triage. All RBRs are required to undergo
Triage when they enter the City.
G. Should a minor (17 years old and below) RBR still

register and fill-out a Health Declaration Form (HDF)
at http://hdf.baguio.gov.ph/?
Yes. All RBRs should register. Minors should be registered

by their parents/guardians. Each member of a family
should separately register even if travelling together. This
is done to ensure that we have data readily available for
contact tracing purposes.
H. What if valid Identification Card is not available?
Please secure a Police Clearance from the Police Office

from the Place of Origin, or any other credible document
showing proof that the applicant is a resident of Baguio
City (ex. Brgy. Certificate of Residence issued by the Brgy.
Captain of your resident Barangay here in Baguio).
I. What if we do not have internet access?
Please ask the help of your friends with internet access to

register or request assistance from the RBR Task Force or
the CDRRMO through a hotline that will be posted soon.

J. I plan to drive from Baguio to fetch my relatives who
were stranded in other provinces. We will all return to
Baguio together, what should we do?
1. The stranded relatives to be fetched should secure

their Health Certificates issued by the
City/Municipal/Provincial Health Office, fulfil the
online registration for their schedule of return, and
get a Travel Authority signed and approved by
PNP Regional Director from their Point of Origin,
and
2. For both driver and relatives, register online
at http://hdf.baguio.gov.ph/,
3. The driver should secure a Clearance Certificate
from the City Mayor’s Office of Baguio City A Proof.
The person to be fetched should already have a
confirmed return schedule before a Clearance
Certificate can be issued of residence will be
required.
4. For both driver and relatives, please follow the
procedure for RBRs when coming back to the City.
K. What about those who need to enter Baguio for

essential short trips such as for medical appointments,
government/agency transactions, work-related
transaction, or to get their things left at their boarding
houses?
1. Register online at http://hdf.baguio.gov.ph/

2. Secure a Medical Certificate from the Provincial/City
Health Office in Point of Origin, and a Travel
Clearance from the Mayor (or his/her duly
authorized representative) of the Point of Origin.

3. Upon arrival at checkpoint, present Travel
Clearance as proof of essential travel in Baguio
City;
L. I am a Baguio resident and I need to go to another

province for a medical appointment,
government/agency transaction, work related
transaction, or to get my things left at my boarding
house. What should I do?
Please apply for a Clearance Certificate from the Office of

the City Mayor. Applicants must present their proof of
residence in Baguio City, and proof of their essential travel
to another province.
