The purpose of this sign on register is to provide an auditable trail of compliance that the workers (and other persons) whose names and signatures appear below have
conducted a Job Safety Environmental Analysis (JSEA) on the above stated Task and have -
a) Participated in the development of the above JSEA (with a minimum of 2 workers conducting JSEA) or have undertaken points d, f and g;
b) Identified each step of the Task/Job to be undertaken, identified all hazards associated with each step and have eliminated or implemented means of controlling
identified hazards to an “As Low As Reasonably Practical” risk, using Hierarchy of Control measures and in accordance with site rules/regulations/procedures.
c) Have had the JSEA listed above approved by all nominated persons;
d) Reviewed this JSEA and agree that the Task/Job is safe to undertake;
e) Developed this JSEA with reference to Major Hazards in addition to localised hazards
f) Received all necessary information and instruction relating to the JSEA listed above;
g) Read and understood the requirements of the JSEA listed above.

Personnel whose names and signatures appear below on this register will:
1) Ensure compliance at all times with the requirements, steps and controls documented in the JSEA stated above;
2) Act in accordance with the directives and information contained in the JSEA stated above.
3) Cease work, control and record any step/hazard that has changed in the Task/Job being undertaken that has not been identified in the original JSEA completion
