    DEED OF DONATION OF SHARE OF STOCK

     
     
    KNOW ALL MEN BY THESE PRESENTS:

    This DEED OF DONATION is made and executed at __________, Philippines by and


    between:

    Ms. REGINA T. GONZALES, of legal age, Filipino, married, Director


    of TRIDEL HOLDINGS CORPORATION a corporation duly organized and
    existing under the laws of the Philippines with principal address at 284 Doña
    Anita Bldg., E Rodriguez Sr. Avenue Quezon City, hereinafter referred to as
    the DONOR.
    -in favor of-

    DR. MICHAEL THOMAS T. GONZALES, M.D. of legal age,


    married, Filipino and resident at 19A 567 Wack-Wack, hereinafter referred to
    as the DONEE.
    -WITNESSETH-

    WHEREAS, the DONOR is the sole and absolute owner of shares of stocks of
    PROFESSIONAL SERVICES, INC. (The Medical City) with Stock Certificate Number
    7525, a domestic corporation duly existing and registered under and by virtue of the
    existing laws of the Republic of the Philippines, with principal office at Don Eugenio Lopez
    Sr. Medical Complex Ortigas Ave., Pasig City, totaling to THREE HUNDRED (300)
    common shares;

    NOW, THEREFORE, for and in consideration of the love and affection which
    DONOR has for the DONEE, the former being the son of the latter, said DONOR by these
    presents transfer and convey, by way of DONATION unto the said DONEE, her heirs,
    successors and assigns such shares of stock totaling to THREE HUNDRED (300) common
    shares, as aforesaid, free from all liens and encumbrances.

    The DONOR do hereby state, for the purpose of giving full effect to this
    DONATION, that she has RESERVED for herself in FULL OWNERSHIP sufficient
    properties to support herself in a manner appropriate to her needs and social standing.

    The DONEE does hereby ACCEPT WITHOUT ANY CONDITION


    WHATSOEVER THIS DONATION of such shares of stock and does hereby express his
    eternal gratitude for kindness and generosity of the DONOR.

    IN WITNESS WHEREOF, the DONOR and the DONEE have hereunto set their
    hands this______ day of ________,2021.
