A legal representative, registered with the Office for the Immigration Services Commissioner (OISC)
A solicitor, barrister or chartered legal executive
A commissioner for oaths
A registered charity
You will need to select the above two boxes even if you are not able to provide a photo or photo ID.

A letter to give permission for your records to be sent to you, or your representative
This can be a short letter or statement and it must be original (not a photocopy for example). The letter must
include:

confirmation that you give permission for your records to be checked


if a representative is completing this application on your behalf, confirmation that you give permission
for your records to be sent to them
your signature and the date, within the last 6 months
If you are completing this application for a child under 12, or you have Power of Attorney for the applicant, you
can sign this letter on their behalf. You must provide evidence if you have Power of Attorney or if you are a
Litigation Friend.

If you do not provide any of these documents, we may not be able to make a decision on your application. Please
note that we may occasionally ask you to provide other documents in addition to those listed.
Your application may be rejected if you do not provide all mandatory documents. If your application is rejected
due to documents not being provided, it will not be considered and an administration fee for each person
included in the application will be deducted from your refund.

What happens next


You have chosen the Standard Service. You should send your documents within 15 working days. If you do not
send your documents within 15 working days your application may be rejected.
