    I, SHEINE BEDUYA CLEDERA, Filipino, of legal age, single, with residence and postal address at
    Block 42 Lot 80 Phase 2A-1, Alta Tierra Homes, Barangay Aldiano Olaes, General Mariano Alvarez,
    Cavite.

    That I, after having been duly sworn to in accordance with law do hereby depose and state that:

    1. That I have obtained a housing loan in the amount of SIX HUNDRED SEVENTY-THREE
    THOUSAND PESOS (₱ 673,000.00) from Pag-IBIG Fund secured by Transfer Certificate of Title
    No. _______________________________ which is registered in the name of SHEINE BEDUYA
    CLEDERA particularly located at Block 42 Lot 80 Phase 2A-1, Alta Tierra Homes, Barangay Aldiano
    Olaes, General Mariano Alvarez, Cavite.

    2. That I have complied with the requirements except for the following documents which I undertake to
    submit within the timelines specified below, reckoned from the date of take-out of my housing loan
    account;

    3. That I hereby authorize the Fund to deduct three percent (3%) of the loan amount from the take-out
    proceeds due to me as cash retention for this undertaking
    4. That I have understood that the cash retention shall only be released to me upon compliance of the
    abovementioned deficiency.
    5. That in case I fail to comply with the deficiency/ies within the prescribed timeline indicated above,
    the total loan obligation secured by the subject title shall be deemed due and demandable and/or
    the cash retention shall be forfeited in favor of Pag-IBIG Fund.
    6. That I am executing this Deed to attest the truth of the foregoing facts to whatever legal purpose or
    intent this may duly serve.
