CERTIFICATE OF CONSENT

I, GLADYS JORDAN DORADO of legal age, Filipino Citizen, single


and presently residing at Tacurong, South Cotabato, do hereby depose
and state:

1. That I am of legal age, born on February 24, 1995, at Tacurong City,


South Cotabato, Philippines;
2. That I am willing to contract a marriage with Janwar Silongan, of
legal age, born on January 18, 1994 at Matalam, North Cotabato,
Philippines, which marriage is to be celebrated at Davao City;
3. That I, of legal age, acknowledges that I am capacitated to give my
consent;
4. That I am freely, voluntarily, and deliberately giving my consent to
contract a marriage with the aforesaid;
5. That my consent to contract a marriage with aforesaid is not vitiated,
forced, or in any way compelled and coerced;
6. That I freely give my consent in observing mutual love, fidelity, and
support to Janwar Silongan, as a wife; and
7. That I am executing this certificate for all legal intents.

IN WITNESS THEREOF, I hereby signed this certificate on this 2 nd day


of October, 2017 in the City of Davao, Philippines.
