VALIDATION FOR MAINTENANCE OF 2017 FIRELINE
ESTABLISHED LOCATED AT BRGY. PANGGAO, MUNAI,
LANAO DEL NORTE WITH AN AREA OF 43.78 HECTARES.

Please be informed that the undersigned conducted an ocular inspection on the accomplishment
of Abdullah Mustapha as contractor for the 2019 1 st quarter maintenance of 2017 fireline established
situated at Brgy. Panggao, Munai, Lanao del Norte with an area of 43.78 hectares.
Thus, the said above mentioned name of contractor had deliberately complied the target activities
required for fire line maintenance. The width of the maintenance constructed fire lines was 10 meters by
43,802.37 meters in length runs within the Timber area where the fire line established.
Activities completed were documented and attached herewith.

accomplishment of Abdullah Mustapha as contractor for the 2019 1 st quarter maintenance of
2017 fireline established situated at Brgy. Dalama, Munai, Lanao del Norte with an area of 20.03
hectares.
Thus, the said above mentioned name of contractor had deliberately complied the target
activities required for fire line maintenance. The width of the maintenance constructed fire lines
was 10 meters by 20,826 meters in length runs within the Timber area where the fire line
established.

EMPLOYMENT CONTRACT
KNOW ALL MEN BY THESE PRESENT:
That this agreement made this 1 st day of January 2019 between the Department of Environment
and Natural Resources, Region 10, Cagayan de Oro City represented herein by the PENR Officer
MAMACAYA M. LUCMAN,AL HADJ, who is located at PENRO Tubod, Lanao del Norte, of legal age,
married and resident of Iligan City hereunder called the party of the first part, ABDULLAH
MUSTAPHA, of legal age, single and resident of Dalama, Munai, Lanao del Norte hereunder called the
Second Part.
WITNESSETH
That the party of the firs part and the party of Second Part in connection of the agreement and
covenant herein made by one to the other be these presents entered into contract of work, whereby the
party of the First Part hires and employs the party of the Second Part and in connection with the
Department of Environment and Natural Resources which will performed in CENRO 10-B, Kolambugan,
Lanao del Norte subject to the following terms and conditions to wit:
1. The said part of Second Part agrees to give and furnished his undivided time and service in the
employee of the first part with such capacity as latter may direct for such period within May 1, to
May 31, 2019, exclusive Saturday, Sunday and Legal Holidays in providing and rendering
assistance to the Chief, Enforcement Section with the compensation of PHP a month payable at
the end of the month under the contract. This include:
 Contract of Service for the Maintenance of the established Fireline of PHP
2,337.00 CY 2017, with an area of 58.87 hectares at Munai, Lanao del Norte.
2. It is further agreed by pertains hereto that their contract will automatically terminate, eases and
expires on the last official working hour on the last day of the contract as herein previously
indicated; and
3. That this contract maybe renewed or extended for another period when need arise, subject of the
availability of funds and the good behavior shown by the party of the Second Part.
IN WITNESS WHEREOF, parties have hereunto set their hands this ____ day of March 31, 2019 at
PENRO, Tubod, Lanao del Norte.
