COMPANY BACKGROUND:

Liebherr-MCCtec Rostock GmbH

Liebherr-MCCtec Rostock GmbH founded in 2002 develops and manufactures

ship cranes, mobile harbour cranes, and offshore cranes. Additionally, reachstackers

and components for container cranes are covered in its product portfolio. The plant's

direct link to the Baltic Sea provide the best logistics conditions. Above all for worldwide

sales of large equipment that have load bearing capacities up to 5,000 tons.

INCIDENT:

Liebherr Crane Collapses During Testing on Offshore Construction Vessel

On May 2, 2020 a serious accident has occurred on board of offshore installation

Cosco (Qidong) Offshore-owned vessel ‘Orion 1’ at the occasion of and during crane

load tests. The 145-meter boom crane had a capacity of 5,000 tonnes at a horizontal

outreach of 35 meters, and its maximum lifting height was 175 meters above the

vessel's deck. It was the first time Liebherr had manufactured and installed an offshore

crane of this size. According to the preliminary investigation, the responsible authorities

and experts unanimously agree that a broken crane hook was the cause of the

accident. The broken crane hook is provided by a third-party supplier. 

Heavylift and rigging specialist Ropeblock, who designed the failed hook that led to the

collapse of the crane installation on the vessel Orion 1, said the initial information form

the investigation suggests that the hook's stem broke but said the exact cause remains

unknown. Ropeblock said that the component was manufactured by a certified supplier

familiar with similar and larger components, prior to manufacture the design has been

verified by the authorized notified body.


Carrying out an overload test is a typical procedure in an industry, whereby the

crane is tested for its maximum lifting capacity. The overload test that led to the

accident was planned for to reach 5,500 tons, the crane’s maximum capacity, however

the failure of the crane hook took place at 2,600 tons. Facts indicate that the incident

occurred at a load of around 2,600 tons causing a chain reaction which led to the

accident. Therefore, the design or production error of the Liebherr can be excluded.

PROPERTY DAMAGES OR FATALITIES

A total of 12 people were injured, 2 people were hospitalized and 10 were

treated on site for minor injuries in the incident. According to initial estimates, the

damage amounts to a high double-digit million figure. The crane has suffered significant

damage. The damage to the crane and the vessel were still being investigated.

Considering the extent of the damages, Liebherr are above all thankful that it did not

cause more personal injuries. Their priorities currently lie in fully supporting the

investigating authorities in order to contribute to the quickest possible clarification of the

accident.

ENVIRONMENTAL HARM

As far as the environmental is concern, I can say that this accident does not

cause to much harm or damaged on the environment. Yet the Orion 1 vessel was due

to install 100 wind turbine jackets at the 950MW Moray East wind farm off east Scotland

next month but DEME said it is now looking for a replacement for the work. A

spokeswoman for DEME told Recharge the company is working on a replacement


vessel for Moray East as a first priority, and is also in contact with the Hornsea 2 project

team over options there.

For the safety of the workers on the site, the standard safety protocols on a

manufacturing company should be monitored. Apart from these safety practices, the

company must meet the required clearances for a better performance and to do those

projects safely.

Conduct Inspection / Testing

An accident can occur to a working site any time. Conducting these inspection

and testing can help to prevent these uncalled incidents. During the installation of the

crane, there’s a shortcoming on the part of the inspector to check the load capacity of

the components to be installed that’s result to the crash incident.

Record

Record keeping hold an important part on maintaining the safety inside a working

station. A detailed data record of the past testings and experienced problem and on how

they were resolved can be used on resolving a major accident when it happens.

Safety Drills

Conducting regular drill can help the workers and staff to stay calm and to do

what is needed to avoid the worst scenario in case of an emergency.
