Sir/Madam,
In supersession of Government letter issued vide even no. 21.5.2015, I have
been directed to invite your attention to the subject mentioned above and to state that the
State Government has decided to implement package/implant rates for providing medical
treatment to the Haryana Government employees/pensioners and their dependents in the
approved private Hospitals. The package rates have been worked out for common
procedures in consultation with specialists serving in various departments of Government
Hospitals and after comparing the rates with normal tariff of leading private empaneled
hospitals, CGHS (Delhi NCR rates) and package rates fixed by insurance companies. This
has been done to rationalize the charges across the various institutions. Under these new
guidelines, 1340 (which include already notified 152 packages) packages for major
procedures have been fixed, which include rates of certain implants as well. The details
are given as under
1. Definitions
i) "Hospital" A private hospital approved by the State Government.
i) "Private Medical Colleges" recognised by Medical Council of India (MCI) in
Haryana state and empanelled by the State Government.
ii) "Package Rate" a lump sum amount charged by the empanelled Private
Hospitals for packages/procedures.
iv) "Day Care" Day Care means indoor stay in the Hospital for less than 24
hours which is duly certified by the concerned hospital.
) "NABH" means hospitals possessing accreditation certificate of National
Accreditation Board for Hospitals & Healthcare Providers. It is further
categorised into a. Entry Level NABH b. Full NABH
vi) "JCI mean hospitals possessing accreditation certificate of Joint
Commission International.
vii) Card" means the ldentity Card issued by competent authority of any
department under Haryana Government.
vii) "Card Holder" shall mean a person having identity card.
ix) "Beneficiary" means a person who is eligible for coverage of medical
benefits and holds a valid card for the benefit.
X "Coverage" shall mean a type of person to be eligible as the scheme,
subject to the terms, conditions and limitations.
xi) 1, 2, 3, 4, 5, 7,10 and 14 days package rate means the number
of

gas, drugs, etc. (charges of ICU/CCU inclusive of ventilator as per para C &1
(vii) of Guidelines, is not to be charged extra).
xii) "Package Rate" a lump sum amount charged by the empanelled Private
Hospitals/Private Empanelled Medical Colleges for packages/procedures.
(b) The empanelled hospitals shal provide treatment on 1340 procedures listed
in Annexure-I & lI at the prescribed package rates/implants (wherever implant rates have
been fixed and notified by Gol or State Government). The Hospital will raise the bill on
cashless or reimbursement mode (as the case may be) from the Haryana Government
employees/pensioners and their dependents. These package rates are applicable only for
procedures mentioned in Annexure-l.
A list of fixed 1340 packages/implants (this includes already notified 152

i. Medicines and consumables - Rs. 1750 - per day. This will exclude
high cost injections such as Anti-D, Anti Haemophilic Factors,
Thrombolytic treatment (Streptokinase), Anti Cancer drugs, Antibiotics
(the per day cost of which is more than Rs. 2000/- irrespective of number
of doses), etc.
iv. The high cost injection is defined as an injection, the per day cost of
which comes out to be more than Rs.2000/- after decreasing 30% on
MRP of purchased injection by the hospital. In such case, health facility
shall give undertaking that no cheaper brand of same composition or
molecule is available in their inventory/ stock.

(i) The fixed package of any procedure/surgery mentioned in column 2 as OPD
in annexure 1 shall be applicable as per Government instructions dated 11.08.1992 and
08.06.2005 and amended from time to time.
The PPP mode packages shall only be applicable on Government Health
Institutions in Haryana State.
(k) Private empanelled hospitals/ Medical Colleges will also abide by the
guidelines and packages fixed under Rashtriya Bal Suraksha Karyakaram (RBSK) made
by Government of India, wherever applicable. However, this will not be applicable on
Haryana Government employees/pensioners and their dependents.
The reimbursement under Rashtriya Bal Suraksha Karyakaram (RBSK)
Scheme shall be paid to the hospital by NHM Haryana.
(m) Empanelled hospitals/ Medical Colleges shall register every cancer case and
report under Haryana Cancer Atlas (HCA) wherever applicable.
(n) All earlier instructions related to fixed packagelimplant rates stand
superseded. These fixed package/implant rates shall come into force with immediate
effect.

This issue with the concurrence of the Finance Department conveyed vide
their U.O No. 18130/70/29/2001-FD-1/2019, dated 15.9.2019.

16 Cochlear Implant Rs.5,35,000/- (for implant with 12 channels/24 electrodes with
behind the ear speech processor). Reimbursement shall be
allowed @ 100% in case of children between 1 to 5 years, @ 80%
in case of children between 5 to 10 years and @ 50% in case of
children between 10 to 16 years. 50% of the cost of wearable
components e.g. Speech Processor, Microphone, etc. (excluding
cords, batteries) for the purpose of upgradation and/or
replacement every 3 years, on the advice of two ENT surgeons of
Government/ Approved Private Hospitals.
E. REIMBURSEMENT OF COST OF CPAP/BIPAP MACHINES
