SECTION 01000 – GENERAL TERMS & DEFINITIONS

PART 1 - GENERAL

1.01 DESCRIPTION OF WORK

A. The following general terms and definitions shall apply throughout this
manual:

1. “Company Representative” means a party or parties duly authorized by


NONESCOST to act on behalf of NONESCOST, with whom CONTRACTOR
may consult at all reasonable times, and whose instructions, requests and
decisions shall be binding to NONESCOST as to all matters pertaining to the
contract.

2. “Contractor Representative” means a party or parties duly authorized by


CONTRACTOR to act on behalf of CONTRACTOR, with whom
NONESCOST may consult at all reasonable times, and whose instructions,
requests, and decisions shall be binding to CONTRACTOR as to all matters
pertaining to the Contact.

3. “FACILITIES” means the structures or items being constructed, installed or


repaired by CONTRACTOR pursuant to the Contract.

4. “WORK” means all the work and services to be performed by CONTRACTOR


pursuant to the Contract.

5. “WORK SITE” means all locations at which CONTACTOR performs any


portions of the WORK.

SECTION 01100 – CONSTRUCTION GUIDELINES

PART 1 - GENERAL

1.01 DESCRIPTION OF WORK

The Contractor will be required to provide a full and detailed tender breakdown
upon being awarded the Contract. This tender breakdown is to show the actual
calculations of the preliminaries, the labor, plant and material costs of the
Works, the build-up of measured rates with on-cost and overheads and any
other allowances used to arrive at the accepted contract amount.

1.02 CONDITIONS OF CONTRACT

The Contractor shall allow for complying in full with the following items
which shall be provided fully in accordance with the terms and Conditions of
Contract.

A. Conditions of Contract

Allow for complying in all respects with the Conditions of Contract


(both general and particular conditions). The Contractor is required to
describe and price items having a financial implication, which are not
included elsewhere.

B. Specification

Allow for complying with all aspects of the project Specifications as


required for the proper execution of the works, but for which a
specific requirement is not explicitly identified elsewhere (and give
full details of same).

C. Drawings

Allow for complying with all aspects of the project drawings as


required for the proper execution of the works, but for which a
specific requirement is not explicitly identified elsewhere (and give
full details of same).

1.03 MOBILIZATION

Prior to the start of construction, the contractor must be able to provide the
following items on site and must be satisfactory up to the budget provided:

Access to or Possession or Use of the Site

Allow for any cost associated with complying with regulations


imposed by any statutory authority regarding access to the site.

A. Limitation of Working Space

Allow for complying with any restrictions associated with


complying to designated work and storage areas.

B. Limitation of Working Hours

Allow for any limitation on the use of noisy equipment during


working hours by the local authority.

C. Maintenance of Existing Services On, Under or Over the Site

Allow for establishing the positions of, diverting, protecting,


upholding and maintaining all pipes, ducts, sewers, service mains,
overhead cables etc. during the execution of the Works and making
good any damage due to any cause within the Contractor's control,
including any costs and charges in connection therewith.

D. Existing Nature

Allow for protecting and/or removing any and/or all existing rock
formations, trees, shrubs and vegetation immediately adjacent to or
located on the site, as required.

1.04 CONTRACTOR’S ADMINISTRATIVE ARRANGEMENTS

The Contractor shall allow for complying in full with the following items
which shall be provided fully in accordance with the terms and Conditions of
Contract:

A. Provide for all on-site administration & supervision costs.

The Contractor shall ensure that a sufficient number of suitably


qualified staff are present on site & to provide all necessary
supervision for carrying out the works including an authorized
Contractors Representative. to comply with the requirements of the
Contract.

B. Site Security.

The Contractor shall provide 24 hour site security system and suitably
qualified security personnel.

C. Safety, Health and Welfare of Workpeople.

All for all costs associated with complying with all safety, health and
welfare regulations appertaining to all workpeople (including those
employed by Sub-Contractors) employed on the Site or elsewhere for
the needs of this Contract. The Contractor is responsible to act in
accordance with all local authorities and statutory regulations.

1.05 CONSTRUCTIONAL PLANT

The Contractor shall allow for complying in full with the following items
which shall be provided fully in accordance with the terms and Conditions of
Contract.:

A. Small Plant and Tools.

Allow for providing all small plant, implements and tools, complete
with all consumable and equipment necessary for the proper
execution of the works.

B. Scaffolding.

Allow for providing, altering, and adapting all scaffolding, guard


rails, walkways, toe boards, ladders, etc., necessary for the proper
execution of the works by himself and all Sub-Contractors.

C. Cranes and Lifting Plant (If necessary)

Allow for providing all cranes, hoists, transport facilities, lifting


equipment and all associated consumables for the proper execution of
the works, including those required by any Sub-Contractors.

D. Plant Required for Special Trades

Allow for providing all necessary plant required for special trades
and/or specialist works which have not been allowed for elsewhere
(and provide details of same).

1.06 EMPLOYERS’ FACILITIES & EQUIPMENT

The Contractor shall allow for complying in full with the following items
which shall be provided fully in accordance with the terms and Conditions of
Contract.:

A. Protective Clothing.

Allow for providing all necessary personal protective equipment


(PPE) for the Employer and his staff, consultants and visitors to the
site throughout the duration of the project.

B. Site Office

The site office on site must be properly built with a door and windows
that can be locked to secure documents and other equipment on-site.

It must have the primary units/materials/equipment that composes a


regular office, toilets, dedicated meeting room for the exclusive use
of the Employer & his consultants with equipment namely: printer,
bond papers, stapler, staple wires, etc. & supplying and maintaining
all office furniture, storage cabinets, shelving and the like, for the sole
use of the Employer and his consultant, all in accordance with the
employers requirements and as set out within the Contract
Documents. See ANNEX “A” for the required minimum Employer’s
Facility Site Office.

C. Electricity & Water

The contractor shall first accomplish the Special Request Form


(PME Form 20 of SECTION 01400- CONSTRUCTION FORMS &
DOCUMENTS) together with a letter to the president asking for a
temporary power and water connection for construction purposes.

For temporary Electrical Connection, the contractor shall attach the


proposed electrical sketch plan indicating the riser diagram and
schedule of loads.

The following must be part of the installation:

1. Brand new/calibrated and sealed KWh meter


2. Copper wire, 14 sq.mm. as transmission lines
3. Molded Case Circuit Breaker, 60A, 230V as main disconnecting
device
4. GI or BI pipe, 2” as transmission pole
5. Secondary racks

For temporary Water Connection, the contractor shall provide their


own sub-meter, waterline and other accessories for water connection.

As soon as the permit is approved, the contractor should


prepare/install the necessary water or electrical devices/equipment to
be used during the construction period until it is ready for connection.
The installation must be inspected first by the designated personnel
from the PME office or NONESCOST Staff before it is connected to
the source.

There must have a proper documentation of their water and electricity


consumption. Water and Electrical consumption shall be
charged/deducted from their periodic billing or claims of payment.

Rate/Cost of Water & Electrical Consumption

The rate/cost in using the water or electrical energy is based on the


monthly billing from the mother meter, that is the amount due divided
by the m3 or KWh used.

D. Drainage & sewage provisions, etc.

The contractor is required to provide drainage and sewage provisions,


telephone, internet connection, telefax, to the offices including all
installation, rental, recurring and removal charges and expenses
related thereto.

E. Shaded Carport / Garage

The contractor is required to provide adequate vehicular access to the


building and erecting a shaded carport adjacent to the building, in
accordance with the employer’s requirements and as set out within
the Contract Documents.

F. Office Consumables

Allow for providing all site consumables, stationary and refreshments


for the full duration that temporary office is maintained on site, as per
the employers requirements and in line with the Contract Documents.

G. Office Computer Desktop

Allow for providing a computer desktop/laptop computer with printer


and scanner, all as per the employers requirements and in accordance
with the Contract Documents.

H. Site Transportation

Allow for providing a vehicle for the Employer and his consultants
exclusive use during the duration of the project, including; driver, full
insurance, fuel, cleaning, servicing, repairs and maintenance, all in
accordance with the employers requirements and in line with the
Contract Documents.

1.07 CONTRACTOR’S FACILITIES

The Contractor shall allow for complying in full with the following items
which shall be provided fully in accordance with the terms and Conditions of
Contract.:

A. Workers Facility.

Allow for providing and maintaining adequate accommodation and


buildings , including site offices, compounds, stores and mess rooms,
complete with all necessary utility provisions, hardware, software and
furniture.

B. Facility Enclosure.

Allow for providing and maintaining adequate accommodation and


buildings , including offices, compounds, stores and mess rooms,

complete with all necessary utility provisions, hardware, software


and furniture

C. Facility Roads/power/lightings/water/ drainage

Allow for providing and maintaining temporary roads, including


hard standings and crossings, water, lighting, power & drainage for
the works.

D. Safety Signs and Signage

Allow for providing construction safety signs and signage to


maintain the construction site safe in order to protect the workers,
employees, visitors and passersby. Signage shall be securely placed
in the areas within the work site and shall not obstruct the access or
openings of the facilities.

Please refer to “Annex B” for PRE-CONSTRUCTION


REQUIREMENTS.

1.08 TEMPORARY WORKS

The Contractor shall allow for complying in full with the following items
which shall be provided fully in accordance with the terms and Conditions of
Contract.:

A. Temporary Works.

Allow for all traffic diversions, clear access to and within the site &
providing all temporary fencing, hoarding, gates, footways, guard
rails, gantries, barricading and the like, as required.

1.09 SUNDRY ITEMS

The Contractor shall allow for complying in full with the following items
which shall be provided fully in accordance with the terms and Conditions of
Contract.:

A. Miscellaneous.

Allow for providing samples of all materials for approval,


constructing all necessary mock-ups, providing all labor, plant and
materials for testing of the works, protecting the works from
inclement weather, all necessary measures to ensure to control noise
and pollution levels, maintenance of public and private roads, allow
for complying with all traffic regulations, all statutory obligations,
obtaining all local authority approvals, consents, no objection
certificates as required for the completion of the works and pay all
fees, charges and costs associated with the issue of such
documentation, complying with all master developer regulations and

requirements, providing good quality progress photographs (with


date shown) of the works under

construction. An adequate number of photographs must be taken for


inclusion in the regular Contractors progress report. Allow for the
provision of all operation and maintenance manuals, provision of all
warranties as required, provision of as built drawings. Allow for the
design, supply, erection, maintenance and removal of an illuminated
site notice boards, in accordance with the contract documents and in
compliance with all local authority regulations. Allow for all on and
off site inspections, as required. Allow for all costs in connection
with the provision and maintenance of the latest version of the
electronic document management system (DMS) for the use of the
Contractor, the Employer and his Consultants, as specified within
the employers requirements.

PART 2 - BONDS & INSURANCES

The Contractor is to list hereunder all costs to comply with the necessary
insurance levels required by the Conditions of Contract.:

2.1 Advance Payment Security / Surety Bond

The bond that will provide security to the Owner if the Contractor fails to meet
its contractual obligations.

2.2 Performance Security

This is the monetary or financial guarantee to be furnished by the winning


bidder and shall be posted prior to the signing of the contract.

2.3 Contractors All Risk (CARI)

This provides coverage for buildings and civil engineering projects during
construction, against accidents, resulting in loss, or damage/destruction of
materials, work progress, construction plant and equipment and construction
machinery.
a. Third Party Liability

This covers for the property damage and third party injury or damage
claims.

b. Workmen's Compensation

An insurance providing wage replacement and medical benefits to


employee injured in the course of employment.

c. Contractor's Plant and Machinery

This is the protection against partial or total loss or damage on machines


and heavy equipment used in the construction.

d. Professional Indemnity Insurance

This is to protect professionals working in the construction against a


wide range of scenarios from errors and omissions to defamation and
intellectual property infringement.

e. Motor Vehicles

An insurance that provide financial protection for cars, trucks,


motorcycles and other road vehicles.

PART 3 - START OF CONSTRUCTION

Upon the start of construction, the contractor must be able to present/submit


the following to the owner’s site engineer:

3.1 Work Schedule.

The contractor must submit a work schedule spanning the duration of


construction of the project and be reminded that assessment of their work
progress will be based on the work schedule they have submitted.

3.2 Manpower Schedule

The contractor must submit a manpower schedule spanning the duration of the
entire project and be reminded that daily manpower during the construction
must coincide with submitted manpower schedule.

3.3 Equipment Schedule

The contractor must submit an equipment schedule spanning the duration of


the entire project and be reminded that the daily equipment used during the
construction must coincide with the submitted equipment schedule.

3.4 Material Test

Prior to the start of construction, the contractor must already have conducted a
trial mix where the test result of the concrete cylinder must adhere to the
specification given for the strength of the concrete to be used. The contractor
is required to submit their concrete mix proportion certified by a registered
materials engineer.

3.5 Soil Treatment

A soil treatment must always be conducted prior to any pouring activity that
has a direct contact with the earth.

3.6 Clearing, Layout and Staking

Construction site must be free from boulders, trees, vegetation and other
foreign materials or substances that will hinder the proper execution of the
project. See Specifications for details.

3.7 Reinforcing Bars

Reinforcing bars must be free from rust, dirt and must adhere to the specified
strength requirement. See Specifications for the detailed instruction.

3.8 Forms and Reports

During the construction, the contractor will be required to submit reports for
the proper documentation of the project. See SECTION 01400
CONSTRUCTION FORMS & DOCUMENTS for the set of forms to be
submitted and their descriptions.

3.9 Pictures / Documentation

The contractor is required to take pictures of the progress of work on site that
will be properly stored for documentation purposes. These pictures will also
be attached to pouring permits, reports and their progress billing.

3.10 Weekly Coordination Meeting

A coordination meeting will be held weekly to discuss the progress of the


project and to address issues encountered on site.

3.11 Safety Protocols

The contractor is required to follow all practiced safety protocols in the


construction site. That includes the wearing of complete Personal Protective
Equipment (PPE) and following COVID-19 Protocols as established by the
Department of Public Works and Highways (DPWH).

The contractor is also required to have a safety office present on site from the
start of construction until its completion. Incompliance to this requirement has
a corresponding penalty as stated in the contract

PART 1 - GENERAL

1.01 DESCRIPTION OF WORK

B. Check grades, lines, levels and dimensions shown on the drawings and verify
all site conditions.

C. Report promptly to Company Representative in writing, any errors,


inconsistencies, or omissions and receive clarification before beginning of the
WORK.

D. Assume responsibility for correctness of all grades, lines. Levels, dimensions


and location of all constructed FACILITIES.

PART 2 - PRODUCTS

PART 3 - EXECUTION

3.01 Surveyor: Provide a qualified land surveyor, subject to approval of NONESCOST,


to perform the following:

3.02 Lay-out: Verify lay-out information shown on the drawings, in relations to the
property survey and existing benchmarks before proceeding to lay-out the WORK.
Protect existing benchmarks and control points. Preserve permanent reference
points during construction.

A. Do not change or relocate benchmarks or control points without prior written


approval. Report lost or destroyed reference points, or requirements to
relocate reference points because of necessary changes in grades or locations.

B. Promptly replace lost or destroyed project control points. Base replacements


on the original survey control points.

3.03 Benchmarks: Establish and maintain one permanent benchmark on the site,
referenced to date established by survey control points. Record benchmark
locations, with horizontal and vertical data, on Project Record Documents.

3.04 Existing Utilities and Equipment: Before beginning WORK, verify the existence
and location of underground utilities and other construction.

3.05 Performance: Working from lines and levels established by the property survey,
calculate and measure required dimensions within indicated or recognized
tolerances. Do not scale drawings to determine the dimensions.

3.06 Lay-out Data: Record lay-out data and changes from drawings on “Project Record
Documents”.

1.02 APPROVALS

All approvals shall be made by NONESCOST Representative.

1.03 SUBMITTAL REQUIREMENTS

A. General

1. The Contractor shall submit to NONESCOST a list and submittal schedule


of all shop drawings, product date, technical literature and samples
required by the Contract. List all items by Specification Sections.

2. All submittals shall be in English and be clearly identified with project


name and location and manufacturer’s name. All submittals shall be
received and approved by the CONTRACTOR prior to submission to the
Company Representative. The CONTRACTOR shall indicate his review
and approval by stamping the submittals accordingly.

3. Required Submittals: As soon as practicable, and in ample time to avoid


delay of the work, submit the shop drawings, catalogue information, and
others to NONESCOST for review. Submit the shop drawings, catalogue
information, etc. assembled in neat package and submit all together at one
time, partial submittals will not be considered. Submittals are required for
all items of equipment specified or indicated on the drawings, items
proposed as substitutes for specified materials, and for the items or data
requested by NONESCOST.

4. The CONTRACTOR is solely responsible for errors in the submittals, and


for accuracy of quantities and the dimensions shown in the submittals.

5. The CONTRACTOR shall review submittals for compliance with the


Contract Documents.

6. Deviations from the requirements of Contract Documents:

a. Cite all deviations in writing.

b. NONESCOST review does not constitute approval of any deviation


unless so stated.

c. Regardless of NONESCOST action on such deviations the


CONTRACTOR is solely responsible for proper fitting and
relationship of all work to adjacent and related work.

Deviations from the requirements of Contract Documents:

7. Date each submittal and identify with the project name and the
specification section covering the submittal. Use numbered transmittal
forms for ease of record keeping, and do not group items covered by more
than one section on the same transmittal.

8. Submittals which have not been reviewed by NONESCOST, or which


have been rejected by NONESCOST, shall not be used for ordering,
fabrication or work at the WORK site.

9. Schedule submittals to allow adequate time for NONESCOST review and


for re-submittal if required.

1.04 SHOP DRAWINGS

A. Clearly detail all aspects of work where required by specifications.

B. Number shop drawing consecutively and maintain numbering through


revisions.

C. Submit 3 prints of shop drawings (3 sets).

D. After review, NONESCOST shall return 1-set to the CONTRACTOR,


who shall:

1. Make no further revisions or changes to the approved copy.

2. Provide copies which has been reviewed by NONESCOST for:

a. CONTRACTOR’s WORK site record

b. CONTRACTOR’s office record

c. Subcontractors, as required

E. The WORK Site copy shall be made readily available for NONESCOST
review upon request.

F. Material’s list, product and equipment schedules and manufacturer’s


technical literature:

1. Cover all aspects required by specifications, Pictorial or vague


advertising literature is not acceptable.

2. Submit two (2) collated and indexed.

3. For substitution requests, include complete specifications of the items


to be considered for substitution.

1.05 SAMPLES

A. Provide samples of the exact item required by the drawings and


specifications.

B. Approval of sample shall be in writing, If CONTRACTOR desires an


approval attached to sample, supply additional copies of sample as needed.

C. CONTRACTOR shall maintain one each approved sample at the jobsite.

D. Label each sample accordingly indicating the type, size, color, finish,
model#, BOQ ref., intended use, the supplier/manufacturer, the name who
submitted the sample and the date of submission. The label shall be
attached firmly to the sample and shall be placed visible for the inspector.
Contractor shall use the material sample submittal format –PME FORM
18 (refer to SECTION 01400 – CONSTRUCTION FORMS &
DOCUMENTS) in this manual, and it shall be used every time a sample
is submitted for approval.

E. Use a separate package/box/pouch for each material.

F. For smaller samples, use a box or thick re-sealable bag to secure the
samples.

1.06 SPARE PARTS

A. Contractor shall provide spares for the materials or fixtures used in the
construction. These materials shall be made available as reference for
purchase and to cover minimal works in case there is a need for immediate
repair of the building.

B. The number of Spares of each type of materials/fixtures used in the actual


construction shall be in accordance to the requirement of the Owner and
shall be stored in a stockroom provided by NONESCOST.

1.07 OPERATIONS AND MAINTENANCE INSTRUCTIONS

A. Collect data as job progresses.

B. Complete data in manual form. Data shall be collated, indexed and bound
in durable covers and identifying on the cover:

1. Project name and date

2. Name, address and telephone number of the CONTRACTOR and sub-


contractor.

Include the name, address and telephone number of the manufacturer


and local distributor with the information of each component,
sufficient for NONESCOST future purchasing.

C. Include only those catalogues and brochures dealing with the products and
equipment used in this project. Remove pages of purely advertising nature,
and pages dealing with items and models not used in this project. Where
more than one item or model is listed on the page, carefully identify the
exact item or model used.

1. Manufacturer’s specifications for each piece of equipment, including


a listing of the recommended spare parts, specialized tools and
preventive maintenance (P/M) schedules.

2. Installation instructions for all equipment; and

3. Details on all tie-ins to existing equipment.

D. Operating Instructions

The following shall be provided:

1. A complete set of start-up and operating instructions.

2. Operator training materials.

E. Submit 3 copies minimum to NONESCOST Representative for review and


distribution. Supply additional copies for review if record copies are
desired by the CONTRACTOR and his sub-contractor.

1.08 AS-BUILT DRAWINGS

A. The CONTRACTOR shall maintain one complete set of drawings at the


site on which, as the work progresses, the CONTRACTOR shall record the
following information relative to work included in this Contract.

1. Changes in size, depth or position of the foundations.

2. Changes made in the structural phases of the work above and below
ground required because of the modifications or Change of Orders.

3. Exact location and elevation of all underground piping and conduit and
the position of all fittings.

4. Changes made in mechanical and electrical work either in location or


detail of work whether above or below ground floor level.

B. On completion of the job, the CONTRACTOR shall deliver one print of


such corrected drawings to the Company Representative. The
CONTRACTOR shall make such additional corrections as the Company
Representative may require and shall then deliver the corrected prints to
Company Representative.

C. The CONTRACTOR is cautioned that these AS-BUILT drawings are a


specific requirement and that the Contract will not be considered complete
until they have been submitted in acceptable form.

1.09 SUBSTITUTIONS

A. Wherever catalogue numbers and specific brands or trade names followed


by the designated material, product, thing or service mentioned in these
specifications, they are used to establish the standards of quality and utility
required.

B. Substitutions which are equal in quality and utility to those specified will
be approved, subject to the following provisions: all substitutions must be
approved by Company Representative in writing.

For this purpose, the CONTRACTOR shall submit to Company


Representative, within 30 calendar days after the effective date of the
Contract, four (4) copies of a typewritten list containing:

1. A description of each proposed substitute item or material.

2. Sufficient data, drawings, samples, literature or other detailed


information as will demonstrate that the proposed substitute is equal
in quality and utility to the items or materials specified, shall be
appended.

Such approval shall not relieve the CONTRACTOR from complying


with the requirements of the Drawings and Specifications and the
CONTRACTOR shall be responsible at his own expense for any
changes resulting from his proposed substitutions which affect other
parts of this own work or the work of other CONTRACTORS.

C. Required list shall be organized as a schedule:

1. List by specification sections.

2. List all items to be used, including specified items and proposed


substitutions.

3. For submitted items, enumerate characteristics differing from


specified items.

4. Effect of proposed substitutions must be reflected in submittals for


other items.

D. Alternate items installed without NONESCOST approval shall be replaced


with the specified material at the CONTRACTOR’s expense. The
CONTRACTOR assumes full responsibility that alternate items or
procedures shall met the job requirements and is responsible for cost of re-
design and of modifications to this and other parties to the work caused by
alternate items furnished under work of this section. No more than one
proposed alternate would be considered for each item.

1.10 GUARANTEES

A. Contractors in the construction industry are required to provide various


forms of guarantees when they are awarded a contract. The purpose of the
guarantees required is to provide Employer with security in the event of
default or non-performance by the Contractor. Refer back to SECTION
1100 PART 2 (BONDS & INSURANCES).

DIVISION 1 - GENERAL REQUIREMENTS


SECTION 01400 – CONSTRUCTION FORMS & DOCUMENTS

PART 1 - GENERAL

1.01 DESCRIPTION OF WORK

Written and graphic documents prepared for communicating the project design
for construction and administering the construction contract.

A. Daily Activity Report (DAR) – PME FORM 01

Daily activity report is a daily log of activities done on-site including Sundays.
It also includes the recording of the weather, equipment used and manpower.
Problems encountered are noted, as well as the person it was referred to. Take
note that every DAR must have the signature of the site engineer. Submission
of DARs’ can either be everyday or at the end of the week, as discussed by the
contractor’s site engineer with the owner’s project in-charge.

B. Workers’ Record Log (WRL) –PME FORM 02

The contractor is required to submit a workers’ log indicating the names of all
their workers that come and go in the construction site. The log must contain
the name, designation, address, age and contact information of all their staff,
personnel and workers.

C. Materials Approval Form (MAF) –PME FORM 03

Materials approval form is submitted by the contractor together with the


materials they need an approval for, mostly, electrical fixtures and plumbing
fixtures. That is, door knobs, switches, light bulbs, outlets, mirrors, lavatory,
sink, water closets, urinal, faucets, doors, windows and other materials needed
for the completion of the project. An approval for materials must be done first
before its installation. The owner has the right to reject materials that does not
follow the given specification and are proven to be of low quality.

D. Materials Delivery Report (MDR) –PME FORM 04

Materials delivery report is submitted by the contractor to the owner’s quantity


surveyor (QS) for inspection. Indicated in the form is the signature of the
person who requested the materials at the same time, the signature of the person
who checked the materials upon arrival on-site. The approved Material Sample
Reference Number must be shown in the form and a copy/copies of the
approval shall be attached. The said report will then be forwarded to the PME
office for recommendation and approval. MDR is done per date of arrival of
materials on-site.

E. Request for Information (RI) –PME FORM 05

Request for information is submitted by the contractor to the owner’s site


engineer to answer their queries regarding the project.

F. Work Request (WR) –PME FORM 06

Work request must be submitted by the contractor’s site engineer to the


owner’s project in-charge prior to any activity that is to be done on-site. That
is, excavation works, soil treatment, formworks, removal of forms, CHB
laying, skim coating, painting, ceiling works, electrical works, plumbing
works, FDAS, installation of doors and windows and any other activities that
requires the approval of the owner’s site engineer. WR must be given to the
owner’s site engineer a day or two (2) before the commencement of the
activity.

G. Request for Inspection (RFI) –PME FORM 07

Request for inspection is a form that is given to the owner’s site engineer a day
or two (2) prior to a major activity that requires an inspection. That is, pouring
activities, steel works, installation of roofing, installation of ceiling,
installation of doors and windows, and any other activities that has a significant
impact on the stability, aesthetics and completion of the project.

H. Pouring Permit (PP) –PME FORM 08

Pouring permit is acquired by the contractor’s site engineer a day or two (2)
prior to pouring activity. Attached to the pouring permit is RFI and pouring
checklist. A before and after photos for the said pouring activity will later be
attached to the pouring permit.

I. Pouring Checklist (PC) –PME FORM 09

Pouring checklist is a list of materials required for a pouring activity to make


sure that all items used for the said activity follows the specifications given and
to avoid delays once the activity has already started. PC must be attached to a
pouring permit upon the acquisition of the latter.

J. Material Test Result (MTR) –PME FORM 10

The contractor is required to submit a material test result a day after the testing
of materials and concrete cylinders are done. The report must show the strength
required and actual strength of the materials and concrete cylinder. In case that
the result of the test is not satisfactory, a letter with the contractor’s
recommendation to avoid reaching the same unsatisfactory result for the next
test must be attach in the said report. For the next succeeding material test
result, the previous submitted report will just be updated. Note that upon the
submission of the report, the actual test result from the testing center must also
be attached.

K. Site Instruction (SI) –PME FORM 11

Site instruction is given by the owner’s site engineer to the contractor in case
there are discrepancies in the plan and specifications that needs a final decision.

L. Non-Conformance Report (NCR) –PME FORM 12

The Non- Conformance Report is a document that addresses specific


deviations or work that fails to meet the quality standard. This document is
created to allow the Contractor to take action to correct the nonconformity and
to eliminate the cause.

M. Submittal Form (SF) –PME FORM 13

The Submittals are documents exchanged during a building project. This is


considered to be the preliminary step in the building process, sent with the
expectation of forthcoming revisions or a request to be answered therein. This
may be accompanied with items such as sample materials, plans, calculation
data, etc.

N. Transmittal Form (TF) –PME FORM 14

Transmittals are documents sent as records, documentation or as requested


documents. This does not require any further response.

O. Punch List (PL) –PME FORM 15

Also called snag list or deficiency list. This list will be sent to the Contractor
listing work that does not conform to contract specifications, the tasks and
items that need to be fixed or completed before a construction project can be
considered finished.

P. Punch Listing Tracker –PME FORM 16

A tracker listing all the items in the Punch List. This helps the inspectors to
track down the list of rectification works and the completion of such. This is
an attachment of the PL.

Q. Defects Rectification Form (DRF) –PME FORM 17

The contractor is required to rectify or make some reworks in the works which
was not apparent or would not have been disclosed upon reasonable inspection
at the time of issuing the final certificate or in any case that the defects may
occur under given circumstances. The defects that had been identified after
final completion of the project shall be notified to the contractor with the
specific details of the defect, what actions to be taken and the timeframe.

R. Material Sample Submittal –PME FORM 18

The contractor is required label each sample accordingly indicating the type,

the name who submitted the sample and the date of submission. The label shall
be attached firmly to the sample and shall be placed visible for the inspector.

S. Gate Pass –PME FORM 19

To prevent the theft or unauthorized carry of materials from the site,


contractors are required to secure a gate pass upon entry or exit to and from the
site. This is also to provide a documented guideline to handle returnable and
non-returnable items especially during the demobilization of the project. No
items/tools/equipment may enter or exit the site without the approved Gate
Pass.

T. Special Request Form –PME FORM 20

Special Request Form must be submitted by the contractor to the President of


NONESCOST for approval prior to any activity/work that is to be done on-
site. That is, request for temporary water or electrical connection, request to
operate a canteen or any request that needs to be approved by PME and to the
Admin of NONESCOST.

U. Routing Slip –PME FORM 21

The main purpose of the routing slip is to ensure that there is specification
when it comes to the route and/or direction in which an information or a
document must circulate.

V. Concrete Sampling and Slump Test Form –PME FORM 22

This is to ensure that the concrete to be used at site is consistently fresh before
it sets. This will also check the workability of the freshly made concrete, and
therefore the ease with which the concrete flows. This can will be an indicator
of an improperly mixed batch. This test must be performed every time there is
a concrete pouring activity. The contractor shall supply all the required fields
and attach photos of the slump test. This is an attachment to the Pouring Permit.

SECTION 01500 – BILLING PROCEDURE

PART 1 - GENERAL

1.01 GENERAL REQUIREMENTS

The following are the general requirements that must be submitted every time
the contractor requests for their billing:

1.01.01 Letter Request

A letter with the intent of requesting for the contractor’s progress


billing signed by the Owner/Manager.

1.01.02 Contract

Contract of the project must be attached to the letter request.

1.01.03 Notice of Award

Notice of award must be attached to the letter request.

1.01.04 Notice to Proceed

Notice to proceed must be attached to the letter request.

1.01.05 Progress Photos

Photos taken from site must be attached to the letter request.

1.02 FOR MOBILIZATION

The following are the requirements that must be submitted upon the request
for payment for mobilization:

1.02.01 Letter Request

A letter with the intent of requesting a down payment or advance


payment signed by the Owner/Manager.

1.02.02 Surety Bond

The contractor must present a surety bond upon the submission of


their letter request for down payment or advance payment.

1.03 FOR PROGRESS PAYMENT

The following are the requirements that must be submitted upon the request
for progress payment:

1.03.01 SWA – Statement of Work Accomplished

An assessment of the work accomplished by the contractor must


be done by their site engineer and will be submitted for validation
by the owner’s site engineer. The assessment done by the owner’s
site engineer and approved by the TWG will be used as final
billing progress of the contractor.

1.03.02 Progress photos

Every payment request must have a corresponding progress photo


attached to it.

1.03.03 Bar chart

A bar chart indicating current status of project accomplishment


must also be attached to the payment request.

1.04 FOR FINAL PAYMENT

The following are the requirements that must be submitted upon the request of
the last and final payment:

1.04.01 SWA – Statement of Work Accomplished

Last and final SWA that indicates a 100% work accomplishment


must be attached to final request.

1.04.02 Progress photos

1.04.05 Liquidated Damages Report

In case the contractor has incurred liquidated damages, a


liquidated damages report prepared by the PME Office must also
be attached to the contractor’s final billing.

1.04.06 Final Acceptance

An acceptance letter prepared by the PME Office and duly signed


by all implementing individuals, end user and the College
President must also be attached to the contractor’s final billing.

1.05 STEPS IN EVALUATING CONTRACTOR’S WORK


ACCOMPLISHMENT (SWA)

1. The Contractor’s Representative will submit their draft work accomplishment


(SWA) on or before 28th day of the month to the PME Project in-Charge (PIC).
Attachments will be as follows:

a. Bar chart.
b. Progress Photos.
c. Highlighted drawings showing the work done.
d. Computations on how they come up with the claim.

2. The PME PIC will forward the draft SWA with its attachments to the PME QS.

3. PME QS will evaluate the contractor’s claim based on the actual progress at
site. He will conduct physical check and quantification of works applied on the
project by the contractor in accordance with the approved plans, bill of quantities
and specifications on a specified period covered. PME QS will then determine
whether Contractor’s claim is reasonable or not.

4. If the Contractor claims for Materials On-Site, PME QS will check and verify
the actual materials delivered at site and validate its actual stock and availability.
PME QS will check the MDRs (Material Delivery Report) that the Contractor had
submitted. MDRs must be approved.

5. In checking the quality of work, PME QS will refer to the approved RFIs. He
may ask the support of PME QA/QC and PME Safety Officer in case there are
deductions that need to be reflected in the claim. The evaluated SWA will then
be forwarded to the M&E Head for review/approval. In case that PME QS’
computation is less than the Contractor’s claim, he must present his computation
to the M&E Head. The PME signatories of SWA will be as follows:
a. PME PIC
b. PME QS
c. M&E Head
d. PME Director

6. PME PIC will send the PME’s official evaluation to the contractor. He may sit
with him for clarification.

7. Contractor will send his revised SWA based on this evaluation.

8. The assessment done by the PME and approved by the M&E Head will be used
as final billing progress of the contractor.

9. Certification of inspection of the physical accomplishment will be submitted


by the M&E Head to the PME Director for approval of payment.

10. The PME Director will recommend to the College President for the final
approval of payment.

CONTRACTOR, or an agency employed by CONTRACTOR. NONESCOST
reserves the right to verify the tests with NONESCOST Testing Laboratory or

1.02 INSPECTION AND INSPECTORS

The Company Representative or other persons or firms appointed by


NONESCOST for this purpose will periodically inspect the WORK and
CONTRACTOR will provide facilities and access to the WORK at all times
as required to facilitate this inspection.

1.03 GENERAL REQUIREMENTS

A. Standards: Sampling, preparation of samples and testing shall conform to


applicable ASTM methods, unless otherwise specified.

B. Reports: Two copies shall be furnished to the Company Representative


summarizing the results of each test and inspection. Each report shall
clearly distinguish the type of test, location, the material tested and whether
an original (first) test or a retest, and other related information.

C. Notification: The Company Representative and if applicable,


NONESCOST Testing Laboratory or agency, shall be notified 48 hours in
advance of any work requiring testing or inspection.

D. Cooperation: The Testing Laboratory shall prepare, cure, store, etc., all
job-made samples.

E. Identification: Materials passing test shall marked or tagged by the Testing


Laboratory and shall thereafter be kept separate and stored so as to
facilitate identification. Any such materials not identifiable by the
Company Representative shall be rejected and removed from the storage
location or the site.

F. Non-conformance: Where a test or inspection reveals non-conformance or


failure to meet requirements, the Company Representative shall
immediately be notified, verbally and subsequently by written report or
letter.

1.04 SCHEDULE OF TESTS

Tests shall include, but not limited to, those specified in further Sections of
these specifications.

1.05 INSPECTION BY THE COMPANY REPRESENTATIVE

A. Notification: In addition to tests and inspections called for in this Section,


CONTRACTOR shall notify the Company Representative for inspection
and testing of various parts of the work as called for in applicable sections
of the Specifications. Notify forty eight (48) hours in advance to assure
inspection prior to covering up or closing in of the work involved.

B. Required inspection shall include, but not be limited to, the following (see
applicable sections of specifications for details):

1. Layout of centreline of road.


2. Foundation excavation prior to forms placement.
3. Concrete forms and reinforcing steel, prior to concrete placement.
4. All concrete pouring.
5. Select fill and base course laying and compaction.
6. Layout of PVC piping (Electrical/Non-electrical)

SECTION 01700 – MATERIALS AND PRODUCTS

PART 1 - GENERAL

1.01 DESCRIPTION OF WORK

Work in this Section includes the minimum standards set forth for materials
and products described in detail in subsequent Sections.

1.02 QUALITY ASSURANCE

A. Specified materials or products establish the minimum technical standards


and required quality.

B. In every case, materials and products shall be new, without defects, and
products shall be received in factory-sealed containers. Materials and
products shall be stored as described in each Section of these
Specifications.

C. Defects shall cause rejection of the product or material, even if earlier


approved by the Company Representative.

Any approval by NONESCOST shall not relieve the CONTACTOR from


complying with the requirements of the Drawings and Specifications and
the CONTRACTOR shall be responsible at his own expense for any
changes resulting from his proposed substitutions which affect other parts
of his own work or the work of other contactors.

1.03 STORAGE AND PROTECTION

A. Prior to delivery of any material to the job site, the CONTRACTOR shall
provide the manufacturer’s recommended method of handling and storage
which shall become the basis for inspecting and accepting or rejecting
actual storage and handling methods used on the work.

B. CONTRACTOR shall properly store products per these specifications and


per manufacturer’s instructions to prevent damage.

C. Store flammable materials, such as paint materials, cleaning materials,


rags, etc. in closed containers, in well-ventilated locations approved by the

Company Representative. Do not store in passageways or next to heating


or electrical equipment.

1.04 MANUFACTURERS

comprehensive or exclusive. Nor shall it be construed as limiting or excluding
submittal for NONESCOST review and approval alternative manufacturers or
equal materials supplied by non-listed manufacturers.

The list of manufacturers is intended solely to identify manufacturers who may


be able to supply specified items or equal. Materials produced by listed
manufacturers, which do not satisfy the requirements of the specifications,
shall not be used. All manufacturers must be approved by NONESCOST.

SECTION 01800 – PROJECT CLOSE-OUT

PART 1 - GENERAL

1.01 DESCRIPTION OF WORK

A. Substantial Completion: Before requesting inspections for Mechanical


Completion, CONTRACTOR shall complete the following:

1. Submit specific warranties, workmanship bonds, maintenance


agreements, final certifications and similar documents.

2. Submit record drawings, maintenance manuals, final project


photographs, damage or settlement survey, property survey, and
similar record information.

3. Complete start-up testing of systems. Remove temporary facilities


from the site, along with construction tools, mock-ups, and similar
elements.

4. Complete final clean up. Touch-up and repair and restore marred
exposed finishes.

B. Record Document Submittals: Do not use Record Documents for


construction purposes; protect from loss in a secure location; provide
access to Record Documents for the Company Representative’s reference.

C. Record Drawings: CONTRACTOR shall maintain a clean, undamaged


set of blue or black line whiteprints of Contract Drawings and Shop
Drawing. Mark-up these drawings to show the actual installation. Mark
whichever drawing is most capable of showing conditions accurately. Give
particular attention to concealed elements that would be difficult to
measure and record at a later date. Organize record drawing sheets into
manageable sets, bind with durable paper cover sheets, and print suitable
titles, dates and other identification on the cover.

D. Record Specification: CONTRACTOR shall maintain one copy of the


Project Manual, including addenda. Mark to show variations in actual
work performed in comparison with the Specifications and modifications,
give particular attention to substitutions, selection of options and similar
information on elements that are concealed or cannot be readily discerned
later by direct observation, note related record drawing information and
Product Data. Upon completion of the work, submit record specifications
to the Company Representative.

E. Maintenance Manuals: CONTRACTOR shall organize maintenance data


into sets of manageable size, bind in individual heavy-duty 2-inch, 3-ring
vinyl-covered binders, with pocket folders for folded sheet information,
and mark identification on front and spine of each binder. Include the
following information:
1. Spare parts list.
2. Copies of warranties.
3. Wiring diagrams.
4. Recommended “turn around” cycles.
5. Inspection procedures.
6. Shop Drawings and Product Data.
7. Fixture lamping schedule.

F. Operating and Maintenance Instructions: CONTRACTOR shall arrange


for the installer of equipment to meet with Company personnel to provide
instruction in proper operation and maintenance. Include a detailed review
of the following:

1. Maintenance manuals.
2. Spare parts and materials.
3. Tools
4. Control sequences.
5. Hazards.
6. Warranties
7. Maintenance agreements and similar continuing commitments.

G. Maintenance Demonstration: As part of instructions CONTRACTOR


shall demonstrate the following procedures:

1. Start-up and shutdown. (Street Lighting)


2. Storm Water Operations.
3. Safety Procedures.

PART 1 - GENERAL

1.01 DESCRIPTION OF WORK

Work in this Section includes, but is not limited to, removal and disposal of
construction debris and miscellaneous existing debris from the site during and
after construction until handing over. Such debris includes, but is not limited
to, all useless materials, rubbish, rocks, stones, excess/waste asphalt and
concrete, excess/waste materials, shipping materials, rebar, and other metallic
debris generated by the project or placed by CONTRACTOR, inside and
outside the WORK SITE.

PART 2 - PRODUCTS

(Not Applicable)

PART 3 - EXECUTION

3.01 CLEANING-UP

A. During the progress of the WORK, the WORK Site shall be kept clean. All
rubbish, surplus materials, and unneeded construction equipment shall be
removed, and all damage repaired so that inconvenience is reduced to a
minimum. The WORK Site shall be kept in a sanitary condition.

Decontaminate the premises and grounds as necessary to prevent the


breeding of insects or vermin.

B. Where material or debris has washed, flowed into, or been placed in


drains, pipes, or structures, whether existing or installed under this
Contract, such material or debris shall be entirely removed and
satisfactorily disposed-off. The drains, pipes, structures, etc., shall be left
in a clean and neat condition.

C. On or before the completion of the WORK, CONTRACTOR, unless


otherwise specifically directed or permitted in writing, shall:

1. Remove all temporary portable cabins and structures built by


CONTRACTOR.

2. Remove all temporary works, tools and machinery or other


construction equipment furnished by CONTRACTOR.

3. Remove all rubbish from any ground including stockpile areas which
CONTRACTOR has occupied.

4. Leave the WORK Site and all adjacent property affected by


CONTRACTOR’s operations in a neat and satisfactory condition
acceptable to NONESCOST.

D. CONTRACTOR, upon completions of the WORK, shall deliver materials


and equipment undamaged and in fresh and appearing-new condition.

E. CONTRACTOR shall restore and replace any of NONESCOST property


damaged by hus work, equipment, subcontractors, employees, to a
condition at least equal to that existing immediately prior to the beginning
of his operations. Suitable materials, equipment, and methods shall be used
for such restoration which shall be done as promptly as practicable, as
work progresses, and shall not be left until Project Completion.

3.02 DISPOSAL AND HAULING

A. All construction debris shall be transported off the WORK Site in


accordance with local laws and disposed of to a NONESCOST approved
dump site. If requested by NONESCOST, the CONTRACTOR shall
produce documents to prove that the debris is being disposed to an
approved government site.

B. Waste material shall not be allowed to blow or spill into any public or
private property during transportation.

3.01 PROCEDURE FOR THE ISSUANCE OF CERTIFICATE OF


COMPLETION AND ACCEPTANCE

A. Once the project reaches an accomplishment of ninety five (95%) of the


total contract amount, the Contractor shall request the Inspectorate Team
to make preliminary inspection for Punch List in preparation for the final
turn-over of the project . Punch List will contain, among others, the
remaining works, work deficiencies for the necessary corrections and the
specific time/duration to fully complete the project.

B. Inspectorate Team for Punch List will compose of the following personnel:
1. Quantity Surveyor (QS)
2. Quality Assurance/Control (QA/QC)
3. Project in-Charge (PIC)
4. PME Staff

C. PME Office will then issue the Certificate of Completion (Please refer to
Annex “B” – Certificates for Completion and Acceptance) once the Punch
List had been completed and satisfied. By this time, the Contractor can bill
and claim their 100% Accomplishment.

D. The Inspectorate Team will conduct a final inspection of the project after
the (1) one-year defects liability period. And if found that the project is
free from any defects/deficiencies that are due to poor workmanship, use
of poor quality materials and non-compliance with the Plans and
Specifications of the Contract, the Inspectorate Team will issue the Final
Acceptance Inspection Report (Please refer to Annex “B” – Certificates
for Completion and Acceptance) and will recommend to the Procuring
Entity for the issuance of the Certificate of Acceptance (Please refer to
Annex “B” – Certificates for Completion and Acceptance).
