RE-ENTRY VISA

REQUIRED DOCUMENTS

1) Visa application form duly filled by the applicant. Complete residential and
official (if applicable) address, telephone numbers, name of parents must be
written in the form. In case of minor(s), the form should be signed by both
parents The visa application form can be downloaded from the website
www.ambislamabad.esteri.it or www.italyvisa-pakistan.com/Islamabad/
2) Two color passport-size photographs no older than 6 months, in white
background and un-edited. One photograph must be glued on the visa
application and the second one, bearing on the reverse side the applicant’s
full name, should be attached to the application form with a paper clip
3) Passport valid at least three months from the expiry date of the visa applied
for. This document must have at least two empty pages. All previous
passport(s), if any, are to be attached. In case of lost passport(s), the Police
Report must be attached in original to the form.
4) An affidavit from the applicant (if it is in Urdu, it should be translated in
English) from the applicant detailing the reason for the re-entry visa
application, and giving details of the applicant’s residence in Italy.
5) If the application for renewal has been submitted, but the applicant left Italy
before its issuance, copy of the receipt of the renewal application to the
Questura must be submitted.
6) If the applicant has overstayed or not renewed the permit due to a medical
condition, he/she must provide the original medical certificates from the
beginning of the treatment, issued from the hospital, stating the reason of
the illness and the duration of the treatment.
7) If the applicant was previously staying in Italy for work, he/she must provide
a letter from the employer stating his/her employment
8) If the applicant was previously staying in Italy for family purpose, he/she
must provide copies of the residence permit and spouse/parents staying in
Italy, as well as invitation letter from the mentioned family
9) Copies of Italian residence/identity documents
10) Copy of flight(s) bookings

NOTE: This type of Visa is only for Pakistani citizens legally residing in Italy and
holding a Permesso di Soggiorno (Residence Permit), but are momentarily without
it (lost or stolen, or expired less than 60 days before and whose renewal has been
requested within the appointment time frame). The issuance of this Visa is
subject to the authorization from the Competent Italian “Questura”, which must
must be requested by the Italian Diplomatic representative.

- An application without a complete set of documents will be rejected


- The Embassy reserves the right to ask for additional supporting documents
- Any forged/fake documents will be reported to the relevant Authorities
