    May kindly see the PUC placed opposite to the file received from Mr. NAIMAT ALI, TGT
    (Urdu), EMP ID-20194241 whereas he has applied for issuance of DGEHS medical card. He
    has joined the department on 07/11/2019 and Rs. 650/- is being deducted from his salary
    towards DHS/CGHS contribution w.e.f Dec 2019. In this regard, the following
    documents/papers/affidavit has been attached for the support of issuance of DGEHS medical
    card.

    1. Forwarding letter duly signed and attested by HOS on school letter head.
    2. Four passport size family photos duly attested.
    3. Application form duly attested.
    4. Appointment letter/joining letter.
    5. First page of Service Book attested by HOS.
    6. Latest Salary slip.
    7. Spouse certificate.
    8. Dependency certificate (AFFIDAVIT)
    9. Details of family (Form – 03)
    10. Undertaking regarding applying for the first time from HOS.
    11. HOS has to certify that employee not issued DGEHS card from anywhere.
    12. Undertaking to delete the name of children if they attain age of 25 or got job
    (attested by HOS).

    In view of the above, the file for kind approval from competent authority i.e. DDE (S/E)
    through zone-29 is being submitted.
