SCORE I. Orientation What is the year? 1 1 Season? 1 1 Date? 1 1 Day? 1 1 Month? 1 1 Which is our state? 0 1 Country? 1 1 Town or city? 1 1 Hospital? 1 1 Floor? 1 1 II. Attention & calculation Spell “world” backwards give 1 point for each letter that is in the right 4 5 place (for example, DLROW = 5) alternatively, do serial 7s (ask the person to count backwards from 100 in blocks of 7 i.e. 93, 86, 79, 65). Stop after five subtractions. Give one pint for each correct answer III. Registration Name three objects (for example, apple, table, pen) taking one second to 3 3 say each one. Then ask the individual to repeat the names of all three objects. Give one point for each correct answer. Repeat the object names until all learned IV. Recall Ask for the three objects repeated above (for example, apple, table, pen). 3 3 Give one point for each correct object V. Language Point to a pencil & ask the person to name this object (1 point). Do the 2 2 same thing with a wrist watch (1 point) Ask the person to repeat the following. No ‘ifs’ & or ‘buts’ (1point). Allow only one trial 1 1 Give the person a piece of blank white paper & ask them to follow a three stage command. “Take the paper in your right hand, fold it in half, and put 2 3 it on the floor.”(The examiner gives the patient a piece of blank paper.) Write ‘CLOSE YOUR EYES’ in large letters & show it to the patient. Ask him to or her to read the message & do what it says (give 1 point if they 1 1 actually close their eyes). Ask the individual to write a sentence of their choice on a blank piece of paper. The sentence must contain a subject & verb, & must make sense. 1 1 Spelling, punctuations & grammar are not important (1 point) Show the person a drawing of two pentagons which intersect to form a quadrangle. Each side should be about 1.5 cm. Ask them to copy the 0 1 design exactly as it is (1 point). All the 10 angles need to be present & the two shapes must intersect to score 1 point. Tremor & rotation are ignored

Interpretation 25-30 Normal 18-24 Mild to Moderate ≤ 17 Less correlate with substantial impairment in ADL
