    AFFIDAVIT
    I, KRISTINA BRAVO TAYLOR, of legal age, married, Filipino
    citizen and a resident of Block 2, Lot 2, Apartment 2, Carmen Ledesma
    Village, Tacas, Jaro, Iloilo City, Philippines, after having been duly sworn
    to in accordance with law do hereby depose and say:

    1. That I am a Petitioner in a Visa Conversion Application in relation to


    my foreigner husband, Douglas Glen Taylor before the Bureau of
    Immigration;

    2. That in relation to the above-mentioned application, I have have to


    submit certain papers/documents required by the Bureau of
    Immigration;

    3. That I guarantee under oath that all the documents submitted are
    genuine with no intention to defraud the government or other
    party/ies;

    4. That I am executing this affidavit to attest to the truth of the foregoing


    and for whatever legal purpose it may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 28th day
