LIFTING OPERATION PLAN
Location: SHELL TAGBILARAN TERMINAL Area: P tanks area
1. Gin pole

All Lifting Operations Require The Following To Be Considered But This List Is Not Exhaustive.
Cultural, communication and language difficulties Conflicting tasks in area
Weight, size, shape and center of gravity of load Environmental conditions including weather and
permissible limits
Availability of approved lifting points on load Lighting in the pick-up and lay-down areas;
Method of slinging/attaching/detaching the load Proximity hazards, obstructions, path of load
Working under suspended loads
Overturning/load integrity/need for tag lines
Access and emergency escape routes
Suitability and condition of lifting equipment to be used Experience, competence and training of personnel
Initial and final load positions and how it will get Number of personnel required for task

Pre-Use Inspection of equipment by operator
Ground and underground considerations
Visibility of the load
Lifting over live equipment Buffer Zone in Effect

Safety officer to check if all activities under preparation listed above have been complied prior to lifting
Ensure the position of the operator shall oversee the spotter or the rigger, area where to lay the materials.
The position of the spotter shall be outside the radius of the boom arm that will be necessary for this activity and safety
officer shall be in charge of this precaution.
Ensure the shackles and sling belt is secured on the gin pole’s hook
Competent person will install the lifting clamp into the shell plates one at a time
Taglines will be installed.
After securing all shackles, lifting clamps and sling belt, gin pole shall carefully lift the shell plate. See illustration
After lifting shell plates, loosen up shackles and sling belt from the hook of the gin pole.
Secure the shell plates by use of sling or stopper.
Clear the area as soon as the lifting is done and inform terminal staff or TM.
