Erection of Second to Six Shell Course (5 shell plates/course)

MATERIAL:
1. Gin pole
2. Slings
3. Shackle
4. Taglines
5. Spreader bar
6. Chain block

Erection of Shell plates (second to six course)

PREPARATION
 Ensure to have an approved permit to work prior to commence activity.
 Wear correct PPE at all times.
 Check the lifting slings for any signs of damage
 Check the correct positioning of gin pole and ensure designated position of personnel involved to be placed.
 Check correct positioning of the loading crew. Rigger in eye contact with the gin pole operator and the crew.
 Ensure everyone is standing at a safe distance.
 Ensure pathway has no obstruction.
 Ensure all non-essential personnel are clear of the area.
 Barricade and put caution tape on the area of lifting.
 Ensure to put warning signs for the lifting activity.
 Ensure to have a standby fire extinguisher & gas tester during activity.

TEST LIFT OF GIN POLE
 Safety officer to check if all activities under preparation listed above have been complied prior to lifting
 Ensure the position of the operator shall oversee the spotter or the rigger, area where to lay the materials.
 The position of the spotter shall be outside the radius of the boom arm that will be necessary for this activity and safety
officer shall be in charge of this precaution.
 Ensure the shackles and sling belt is secured on the gin pole’s hook
 Competent person will install the lifting clamp into the shell plates one at a time
 Taglines will be installed.
 After securing all shackles, lifting clamps and sling belt, gin pole shall carefully lift the shell plate. See illustration
 After lifting shell plates, loosen up shackles and sling belt from the hook of the gin pole.
 Secure the shell plates by use of sling or stopper.
 Clear the area as soon as the lifting is done and inform terminal staff or TM.

3. Has a pre-use inspection of the lifting equipment been carried out and are the lifting

Safe working load
Unique identification number
Valid certification date
4. Are all safety devices working?
5. Does everyone know the person in charge of the lift?
6. Is everyone competent and aware of his/her tasks?
7. Is there a current lift plan and Job Safety Assessment and does everyone understand the
job and precautions?
8. Does everyone know the environmental limits (e.g., maximum permissible wind speed) for
the lift ?
9. Is the lift area controlled and is everyone clear if the load falls or swings?
10. Are signaling methods and communication agreed and clear to everyone?
