MULTI-DISCIPLINARY ZONAL RAILWAY
TRAINING INSTITUTE, DHANBAD (Bhuli)
DETAILS OF ACCIDENT
 DATE – 13th Janurary 2022.
 TIME – 17:00 hrs
 TRAIN – 02346 DN, Saraighat Special.
 TYPE OF ACCIDENT – Derailment (12 coaches got
derailed of which 2 capsized)
 PLACE - New Domohani station under Alipurduar Jn
division of N.F. Railway. The location is about 42
kms away from New Jalpaiguri station and about
100 kms away from New Alipurduar station.

PROCESS ADOPTED
The inquiry committee concluded the
Cause of the accident as follows:
1. Working of train without ensuring
clamping and pad locking of point after
signal failure even though route did not
appear on panel.

WHAT RULE SAYS..
 S. R. 3.68 (b): Stations where Point, Lock and Route
indications are specifically not provided on the panel or
fails to appear in the Panel OR in the event of failure of
RRI/SSI /PI or EI:-
“At stations where Point, Lock and Route indications are
specifically not provided on the panel or it fails to appear
in the indication panel or in the event of failure of
RRI/SSI/PI or EI provided for the purpose of train
operation, the Station Master on duty shall ensure the
clearance of the nominated route, proper setting of all
points through crank handle operation and got all the
points whether facing or trailing clamped and padlocked
on the route.”

2. Rectification work being carried
out on Operating Panel without
disconnection.

WHAT RULE SAYS..
S. R. 3.51 [c]: Disconnection of points and signals. –
Whenever attention of Signalling staff is required for
rectification of points, signals or any interlocking
gear, the Station Master on duty shall issue a memo
to concerned SSE/JE(Signal)/ESM/MSM in a form
prescribed for the purpose. Before taking in hand
any disconnection of points, signals or any
interlocking gear, the person in charge of the work
shall advise the Station Master in writing on Form
S&T (T/351) and obtain his signature before the work
is started and after it is completed.

WHAT RULE SAYS..
S. R. 3.51 [d] : Whenever alterations or repairs are being
carried out to interlock points, signals or any interlocking
gear, action shall be taken as per instructions below –
i. On receipt of advice by the Station Master on duty
from the Permanent Way or Signal and Interlocking
staff, about the work to be taken in hand a restriction
of 15 Kilometres per hour shall be imposed while the
work is in progress and Caution Orders issued in
accordance with “Appendix – ‘A’ until the work is
completed. The Permanent Way or Signal and
interlocking staff in charge of the work shall be
responsible for providing the necessary protection in
accordance with the rules laid down in G & SR 15.06
before taking the work in hand.

WHAT RULE SAYS..
ii. When the work does not necessitate disconnecting
the points or point locks from the levers operating
them, the Transportation staff shall be responsible
for the correct setting and locking of all points by
the appropriate levers before permitting any
movement over them. The Permanent Way or
Signal and interlocking staff in charge of the work
shall be responsible for seeing that the working of
points and point locks by the appropriate levers is
not interfered with in any manner and that all
points over which the movement is to be made are
correctly set and locked.

WHAT RULE SAYS..
iii. When for any reason points or point locks in
station yards are disconnected from the levers by
which they are operated, the Permanent Way or
Signal and Interlocking staffs who makes the
disconnection shall set the points as required, for
traffic movement. It shall be the responsibility of
the Transportation staff to secure such points by a
clamp or bolt and cotter and padlock after
satisfying themselves that the points are correctly
set, as required before permitting any movement
over them.

3. Defective button circuit of Chaygaon
yard(CGON) causing undue operation of
Point 106/108.

WHAT RULE SAYS..
 Any failure in panel should be immediately
reported to the S & T department of station in
written with acknowledgement. The defective
gears should not be used until rectification of the
gears has been ensured by S & T department.

RESPONSIBILITY..
 All the concerned staff four persons
viz SS/CGON, Sr. Tech/Signal / CGON,
Khalasi / Helper / signal /COON,
MS/Porter / CGON were working
Hand in glove in connivance with
each other resorting to shortcuts and
unsafe working."

LESSON TO BE LEARNT…
 Shortcut method and unsafe practices should be
discouraged.
 All frontline staff must have proper knowledge of
rules.
 All failures should be immediately reported to
concerned departments.
 Correct setting and locking of points must be
ensured before allowing any train movement over
line.
