Accomplish the application form in duplicate. Type or print completely
and clearly, all information required in the forms.

Only the owner of the business is authorized to sign all the forms. A
representative may sign for and in behalf of the owner
provided a Special Power of Attorney authorizing the representative is
submitted.
4.
Submit the following (where applicable) together with the application
form to the proper DTI Office where your business is located.
New Application
Renewal Application
a.Two (2) identical passport size picture (with signature

good morning po, i just wanna ask lang po why my applicastion is
still on your office, i was just called this morning on air 21 to
check if they're still have my approval application formr? and
they'd answer me no there's nothing' they not have it yet pa po,
they told that it's still in you'r office, why it took long that
much? you told in you'r office aftern i applied it that it was yor
style now on approving application, and you told me also that it's
the easiet way on applying application, tha you will the onr to send
us, this is my comment now....... mas lalo nyo pong pinatagal at
pinabagal ang proseso kse bakit ko po nasabi....?????? hanggang
ngayon po e wala pang dumarating saken na galing air 21 next week ay
mag iisang buwan na po, i was applied it march 02, we'r now coming
to the end of the month, but i was'nt receieving it yet untill now,
it still there in yor office, why it took that long????....... mas
mabuti pa yung nakaraan ninyong style, mabilis lang last year po
kase nag apply yung friend ko that same day nakuha nya rin, ibalik
nyo nalang po sa dating style nyo last year na ganun kadali one day
processing lang kahit medyo mahal dka naman po parang nag aantay sa
wala............. yun lamg po at maraming salamat po!
