    PROTEST OF BILL OF EXCHANGE

    BE IT KNOWN that on the _________ day of _________, 2___ at _________ A.M., in


    the city/or municipality of ____________________, Philippines, at the request of
    __________________, the holder of the draft hereinafter set forth, I ___________, a duly
    commissioned Notary Public for and in _________________, personally presented the original
    of said draft at ____________to the drawee, and demanded payment (or acceptance, as the case
