Pakyaw Agreement

This agreement is between the Department of Education and the Pakyaw Group for labor services. The Pakyaw Group, led by Mario B. Andresio, will provide carpenters and laborers to mow the school front yard and soccer field for Php 1,300.00. They must complete the work within 2 days under the supervision of the site supervisor. The Pakyaw Group, led by Allan T. Sabellano, will also provide rewiring, installation of ceiling fans, outlets, switches and buzzers for Php 9,500.00 and must finish within 8 days.

PAKYAW AGREEMENT

KNOW ALL MEN BY THESE PRESENT:

THIS AGREEMENT, made and entered into this 5th day of February, 2020 at Liberty
National High School by and between the Department of Education, Division of South Cotabato,
City of Koronadal, represented herewith by the Schools Division Superintendent, duly
authorized for the purpose and hereafter to as the implementing agency and the
Electrician /Private Individual /Carpenters/ Laborers/ Welders represented by Pakyaw Group
Leader MARIO B. ANDRESIO with the address Liberty, Tampakan, South Cotabato.

THAT for and in consideration of the rates in the Job Order, to this Agreement and made an
integral part hereof, the parties hereto covenant and agreed as follows:

1. That the Pakyaw Group shall provide carpenters and labors services necessary for the
proper and faithful development of the school the work described hereto as Labor for the
MOWING OF SCHOOL FRONT YARD AND SOCCERFIELD with the amount of
Php 1,300.00 for the operation of LIBERTY NATIONAL HIGH SCHOOL.

2. That the Pakyaw Group shall complete the work in accordance with the corresponding
Scope of Works, under the supervision of the site supervisor appointed for the purpose by
the Department of Education.

3. That the Pakyaw Group agrees to provide with the services within upon signing of the
Contract and to complete the same within 2 days only, inclusive of Saturdays, Sundays
and Holidays.

4. That the Pakyaw Group shall submit to the Department of Education, Division Office, a
periodic report on the time records of the workers, the group accomplishment and
corresponding payment hereon.

IN WITNESS HEREON, the PARTIES aforesaid have hereunto affixed their signatures on the
price above written.

SCHOOL.

2. That the Pakyaw Group shall complete the work in accordance with the corresponding
Scope of Works, under the supervision of the site supervisor appointed for the purpose by
the Department of Education.
