    At pipeline Km 31, the side glass window of the excavator registration no. 2195 JAA, operated by certificate no.
    BHOP002388 was covered with tape (i.e., entire window was block) obstructing the 360-degree view of the operator during
    operation.
    References
    Equipment operators shall have 360-degree visibility when operating equipment (e.g., curtains/cardboard shall not obstruct
    the operator’s view) as per CSM III-2 Mechanical and Heavy Equipment, paragraph 2.3.14.
    Action Taken
    The Contractor removed the tape from the side glass window of excavator no 2195 JAA. Conducted refresh training
    regarding Mechanical and Heavy Equipment requirements.
