    RESOLUTION DECLARING THE LOWEST CALCULATED AND RESPONSIVE QUOTATION


    AND TO AWARD THE CONTRACT “PROCUREMENT OF COVID-19 RELATED SUPPLIES
    (NEGOTIATED PROCUREMENT – SMALL VALUE PROCUREMENT)” TO AVI OFFICE AND SCHOOL
    SUPPLIES TRADING.
    WHEREAS, the Department of Education, Little Baguio Elementary School invited prospective
    suppliers in PHILGEPS on October 01, 2021 to submit Request for Quotation for the contract
    (PROCUREMENT OF COVID-19 RELATED SUPPLIES);

    WHEREAS, in response to the said invitation, five (5) suppliers submitted quotation/s, subsequently
    opened on August 27, 2021, to wit:

    WHEREAS, on the same day, the School Bids and Award Committee conducted an evaluation of
    the quotation presented by the suppliers and proceed for the post qualification and after careful
    examination, validation and verification of the said documents, its quotation was found to be responsive
    and COMPLYING;
    NOW, THEREFORE, we the members of the Schools Bids and Awards Committee, hereby resolve
    as it is hereby RESOLVED:
    1. To declare the quotation of (AVI OFFICE AND SCHOOL SUPPLIES TRAIDING) for the
    contract “(PROCUREMENT OF COVID-19 RELATED SUPPLIES)” as the Lowest Calculated
    and Responsive Quotation;
    2. To award to (AVI OFFICE AND SCHOOL SUPPLIES TRAIDING) the said contract in the
    amount of thirty eight thousand two hundred one pesos only (Php 38,201.00);
    3. To recommend approval by the School Head of Little Baguio Elementary School, the foregoing
    evaluation.
