BENEFITS ELIGIBILITY

Flight Attendants who meet the CBA designation for the Flight Attendant work group, are eligible for full-time insurance
benefits as follows:

 Medical, Dental, Vision, Flexible Spending Accounts and Short-Term Disability begin 1st of month following hire
date.
 All other benefits (Long-Term Disability, Life Insurance, Critical Illness and Accident), not including travel, begin 1 st
of month following or coinciding with 90 days of employment.

MEDICAL BENEFITS
Frontier Airlines offers the choice between 3 medical plans through UnitedHealthcare (UHC). To see a current list of network
providers online, visit www.myuhc.com and select the Choice Plus network. Frontier will contribute approximately 27% of
the premium for the Legacy PPO plan, 70% for the Traditional PPO plan and 79% for the Value HDHP plan. The table below
summarizes the key features of each plan.

DENTAL BENEFITS
Frontier Airlines’ dental coverage will provide you and your family affordable options for overall health. Coverage is available
through Delta Dental of Colorado. Frontier pays 100% of the premium for employee only coverage on the Standard Plan
and 65% of the premium on the Premium plan.

HEALTH SAVINGS ACCOUNT (HSA)


Available to employees enrolled in the Value HDHP plan. An HSA is a personal health savings account that you can use
to pay out-of-pocket health care expenses with pre-tax dollars. Your HSA can be used for qualified expenses, including
those of your spouse and/or dependent(s), even if they are not covered on your medical plan.
In 2022, Frontier Airlines will contribute the following amounts to your established HSA bank account if you elect the Value
HDHP medical plan:
 Employee Only: approx. $75/month
 All Other Tiers: approx. $150/month

For 2022, contributions are limited to the following:


 Individual: $3,650 *
 All Other Tiers: $7,300 *
 Catch-up (ages 55+): $1,000

* The maximum limits referenced above are a combination of employee and employer contributions

FLEXIBLE SPENDING ACCOUNTS


Flexible Spending Accounts (FSAs) allow you to set aside pre-tax dollars to pay for health care, day care, parking & transit
expenses.

HEALTH CARE FSA


You can contribute up to $2,850 on 2022 for qualified health care expenses such as co-pays, deductible, etc. You cannot
participate in this if elect the Value HDHP. You are eligible to rollover up to $550 into the next benefit plan year.

LIMITED PURPOSE HEALTH CARE FSA


If you are participating in the Value HDHP medical plan, you may enroll in this FSA. This plan can only be used to
reimburse dental and vision expenses. You can contribute up to $2,850 for 2022 and you are eligible to rollover up to
$550 into the next benefit plan year.

DEPENDENT CARE FSA


Allows you to set aside money on a pre-tax basis to pay for day care related expenses for children under the age of 13 or
a child over 13, spouse, or elderly parent residing in your house who is physically or mentally unable to care for himself or
herself. You can contribute up to $5,000 in 2022 if you are married and file a joint return or if you file a single or head of
household return. If you are married and file separate returns, you can elect $2,500 for the 2022 plan year. You are eligible
to rollover any remaining balance into the next benefit plan year.
PARKING FSA
Allows you to set aside pre-tax dollars to receive reimbursement for parking expenses incurred while working. You can
contribute up to $280 per month.

TRANSPORTATION FSA
Allows you to set aside pre-tax dollar to receive reimbursement for public transportation expenses such as mass transit
(buses, trains, and ferries) for the purposes of going to and from work. Cannot be used on tolls, Lyft or Uber. You can
contribute up to $280 per month.

LIFE INSURANCE
COMPANY-PAID LIFE AND ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D)
Company-paid life and AD&D are provided at no cost to all eligible employees through The Hartford. If you die as a result
of an accident, your beneficiary will receive both the life benefit and the AD&D benefit which is equal to one times your
base annual salary up to a maximum of $500,000.

VOLUNTARY LIFE
Frontier Airlines provides you the option to purchase voluntary life coverage for yourself up to $500,000 or 5 times your
base annual salary, whichever is less. Guarantee Issue is 5 times your annual salary or $200,000, whichever is less. You
can purchase coverage for your spouse up to 100% of the employee coverage or $250,000, whichever is less. Guarantee
issue for your spouse is $50,000. Child coverage can be purchase in the amount of $10,000. You may only enroll your
dependents if you have elected the voluntary life for yourself.

DISABILITY INSURANCE
Short-Term Disability (STD) benefits are available to you on a voluntary basis. STD insurance is designed to help you meet
your financial needs if you become unable to work due to a non-work-related illness or injury. It replaces 50% of your
monthly average pay up to max of $510 per week. The waiting period for benefits is 7 days and benefits can last up to 26
weeks.
Frontier Airlines automatically provides 100% company paid short-term disability (STD) insurance to eligible Trenton crew
base employees through The Hartford. STD insurance is designed to help you meet your financial needs if you become
unable to work due to a non-work-related illness or injury. • Benefit: 85% of base weekly pay* up to $993/week (Subject to
change annually in accordance with NJ State mandates) • Elimination period: 8 days • Benefit duration: Up to 26 weeks.
Long-Term Disability (LTD) benefits are provided to you at no cost. LTD is designed to help you meet your financial needs
if your disability extends beyond the STD period of 26 weeks. It replaces 60% of your base monthly pay up to $5,000 per
month. The waiting period is 180 days and benefits can last up to five years if disabled prior to age 65, or until returning to
active service whichever is earlier. If disabled between age 65 and 68 the LTD benefits can be received up to age 70, but
not less than one year or until returning to active service, whichever is earlier.

RETIREMENT PLAN
Frontier Airlines encourages you to invest in yourself and your future by participating in the Frontier Airlines, Inc. 401(k)
Retirement Plan through Charles Schwab. You have the option to contribute on a pre-tax basis, post-tax or Roth basis or
the combination of the two.
You can contribute 1% to 90% of your salary on a per-pay period basis. The maximum contribution for 2022 is $20,500. If
you are age 50 or older by the end of the calendar year, you can also participate in the catch-up contribution of $6,500 for
2022.
We have a wide range of investments including Target Date Retirement funds. If you would like retirement plan assistance,
Guided Choice, an independent investment advisory service, can assist.
Our plan accepts rollovers and you can take out loans for purposes outlined by the IRS.
Company matching contribution of 100% up to the first 6% of deferral for Flight Attendants begin after one (1) year of
employment and vest twenty five percent (25%) per year at the start of years two (2) through five (5).

PAID TIME OFF


VACATION
Vacation days are accrued in the current calendar year to be taken in the following calendar year according to the following rates (based
upon years of service as a Flight Attendant as of December 31 st):

SICK

Sick leave is a form of paid-time off that accrues in an account and serves to protect you and your income in the event of
an unexpected, illness, injury, accident or prolonged medical condition that prevents you from working. Sick hours may be
used only for the employee’s own illness or for that of their immediate family members.
A sick bank account is maintained for each Flight Attendant. A Flight Attendant shall accrue sick leave at the rate of 4.00
hours for each month of employment. Sick leave shall be accrued and retained while in a Leave of Absence. A Flight
Attendants sick bank shall not exceed 700 hours.

TRAVEL BENEFITS
Employees, their dependents (Spouse, Child(ren) and Domestic Partners), Parents and Companions have access to travel
privileges on Frontier Airlines after 30 days of employment.

Cost: $1.50 per flight segment

Summary of Benefits:
