JOINT AFFIDAVIT OF LEGITIMATION

KNOW ALL MEN BY THESE PRESENTS:
We, ROMAR T. ARTICULO and MARY JOY G. ARTICULO, both of legal

ages, married to each other, Filipino citizens and residents of Caoigue, Tubao, La Union, after
having been duly sworn to in accordance with law, do hereby depose and state, that:
1. We humbly submit and declare that we are the natural and biological parents of
TRISTAN MAR QUERO GARCIA, who was born on August 13, 2003 at Nazareno
Hospital, Agoo, La Union;
2. Considering that we were not yet legally married although capacitated to marry each
other at that time of birth of our said child TRISTAN MAR QUERO GARCIA, his
Certificate of Live Birth issued by the Office of the Local Civil Registrar of Agoo, La
Union and Philippine Statistics Authority (PSA) indicated that we are not yet married;
as such he is considered illegitimate;
3. Subsequently, on November 16, 2006, we formally celebrated our marriage, such fact
of our marriage was duly registered before the Office of the Local Civil Registrar of
Tubao, La Union and Philippine Statistics Authority (PSA);
4. In view of the foregoing, our child- TRISTAN MAR QUERO GARCIA will now be
considered as legitimated by operation of law;
5. We are executing this Affidavit to attest to the veracity and truthfulness of the
foregoing narration of facts; for the purpose of declaring said TRISTAN MAR
QUERO GARCIA as our legitimated child; and for whatever intents and purposes
this may will serve.

2022 at Tubao, La Union, Philippines.

We, MARK JOSEPH GAYO JARAVATA and JENNIFER KREBS

JARAVATA, both of legal ages, married to each other, Filipino citizens and residents of
Baldios, Sta. Ignacia, Tarlac, after having been duly sworn to in accordance with law, do
hereby depose and state, that:
1. We humbly submit and declare that we are the natural and biological parents of
KALIA NAOMI KREBS JARAVATA, who was born on September 6, 2019 at
Jecsons Medical Center, Tarlac City, Tarlac;
2. Considering that we were not yet legally married although capacitated to marry each
other at that time of birth of our said child KALIA NAOMI KREBS JARAVATA,
her Certificate of Live Birth issued by the Office of the Local Civil Registrar of Tarlac
City, Tarlac and Philippine Statistics Authority (PSA) indicated that we are not yet
married; as such she is considered illegitimate;
3. Subsequently, on January 18, 2022, we formally celebrated our marriage, such fact of
our marriage was duly registered before the Office of the Local Civil Registrar of
Santa Ignacia, Tarlac and Philippine Statistics Authority (PSA);
4. In view of the foregoing, our child- KALIA NAOMI KREBS JARAVATA will now
be considered as legitimated by operation of law;
5. We are executing this Affidavit to attest to the veracity and truthfulness of the
foregoing narration of facts; for the purpose of declaring said KALIA NAOMI
KREBS JARAVATA as our legitimated child; and for whatever intents and
purposes this may will serve.
