JOINT AFFIDAVIT
(RE: LATE REGISTRATION)
We, JOJIEMAR CALALITA BAUTISTA and JENNY INDING SUSON, both of

legal age and married, Filipinos, and both residents of Buenavista, Zamboamga City,
after having been sworn to in accordance with law, do hereby depose and state;
That, we personally know and are familiar with the spouses LEONORA SOLIS
MAGBANUA and ISMAEL DAUNG BARAHIM, of Buenavista, this City, as they are
one of our neighbors and close family friend who gave birth to the child named as
JAMILA MAGBANUA BARAHIM who was born on October 24, 1998;
That, we are likewise familiar with the fact that the event and surrounding
circumstances of child’s birth have not been registered with the Office of Civil Registrar
of Zamboanga City or anywhere else in the Philippines, because of lack of sufficient
time to cause the registration;
That, we are executing this affidavit freely and voluntarily without any
compulsion or intimidation imposed upon us, to attest the veracity of the foregoing
assertions and averments, and as a supporting document for the late registration
of the fact of birth of Mr. JAMILA MAGBANUA BARAHIM, and to pave the way
for the early release of Certificates of Live Birth, and for any lawful purpose it may
serve.

JOINT AFFIDAVIT
We, MARLON S. LUZON and REYNAN F. PAJARUM, both of legal age, married, Filipino,
with residences address at Guiwan and tugbungan, Zamboamga City, respectively, after being
sworn in accordance with law, hereby depose and say, that;
We personally know and are familiar with “JENEVIE S. DUWAT”, she being our neighbor;
‘JENEVEI’ was born on December 11, 1993 at Tugbungan, Zamboanga City and the 2 nd
child of seven children of the spouses JHONNY L. DUWAT and EMMA SINDICO DUWAT;
We are likewise familiar with the fact that the event and surrounding circumstances of
the nativity of ‘JENEVIE S. DUWAT’ had not been recorded in the Civil Registrar of Zamboanga
City or anywhere else in the Philippines, as his biological parents were then pre-occupied
earning for their subsistence, lack of interest and/or due to ignorance, not to mention as well,
was the fact that the “Traditional Midwife (Hilot)” who attended her delivery did never persist
in reporting her birth to the agency concerned;
We are executing this affidavit freely and voluntarily without any compulsion or
intimidation imposed upon us, to attest the veracity of the foregoing assertions and averment,
as supporting document for the late registration of the fact of birth of our neighbor “JENEVIE S.
DUWAT”, to pave the way for the early release of her Certificate of Live Birth, and for any
lawful purpose thereto attendant and appertaining.
Further, we sayeth naught.

We, MARLON S. LUZON and Reynan F. Pajarum , both of legal age, married, Filipino,
with residences address at Guiwan and Tugbungan, Zamboamga City, respectively, after being
sworn in accordance with law, hereby depose and say;
That, we personally know and are familiar with Artemio Orenciada and Evangelyn
Blaquio as they are one of our family friends;
That, it is also known to us that they had lived together as husband and wife, and
subsequently, got married with each other on December 18, 2012;
That, they have blessed with four Children from the time they have lived together,
namely;
1. ARGILYN BLAQUIO ORENCIADA, FEMALE, born on MARCH 24, 1993;

2. ARIEL BLAQUIO ORENCIADA, MALE, born on MARCH 20. 1996;
3. PRECIOUS BLAQUIO ORENCIADA, MALE, born on NOVEMBER 04, 2005; and
4. NIÑO BLAQUIO ORENCIADA, MALE. born on FEBRUARY 20, 2008
That, we are executing this affidavit freely and voluntarily without any compulsion or
intimidation imposed upon us, to attest the veracity of the foregoing assertions and averments,
as supporting documents for the late registration of the facts of birth of the four children
named above to pave the way for the early release of Certificates of Live Birth, and for any
lawful purpose thereto attendant and appertaining.
Further, we sayeth naught.
