    St., Payatas B., Quezon City, after having been duly sworn to in accordance with law, hereby
    depose and state:

    1. That I am the registered owner of a motorcycle which is more particularly described as

    YEAR MODEL. : 2016
    MV FILE NO. : 1303-00000699316

    2. That I overlooked the registration process for the above-mentioned motorcycle. Hence,
    the denomination appearing on the Certificate of Registration and Official Receipt of the
    motorcycle issued by the Land Transportation Office (LTO) was MOTORCYCLE WITH
    SIDECAR instead of MOTORCYCLE (without SIDECAR), which is true and correct one;

    3. That the said discrepancy may have been caused by inadvertence or through an honest
    mistake during the accomplishment thereof;

    4. That I am executing this affidavit in order to attest to the truth of the foregoing
    statements and for whatever legal purposes it may serve;
