These are typically used in NP/PA/MD notes for quick reference to laboratory values. It is also a
good idea for the nursing student to understand this format and be familiar with it, as you may
see it within the medical record. The placement of these laboratory values may differ slightly,
however, most are all “standard.” See below…
An easy way to remember how to fill out this fishbone, is to remember “SALTY BUNS”
(Sodium, Chloride, BUN) or (NaCl BUN). The rest is potassium (K) carbon dioxide (CO2) (also
referred to as the Bicarb). Creatinine (Cr) and then the serum glucose on the right.

A good way to remember how to fill out this fishbone, on a CBC, the white blood cell count
(WBC) usually is listed first, followed by the hemoglobin (Hgb) and the Hematocrit (Hct), with
platelets (Plt) coming last.
