Description of lifting operation: LIFTING / LOADING DRILL PIPE BASKET

NOTE: The crane configuration on 7 falls.

Method Statement
 CRANE ACCESS THE SITE WITH ONE LIFTING SUPERVISOR AND TOW RIGGERS AND A BANKSMAN
 RIGGERS WILL PREPARE BARRICATION AROUND THE AREA UNDER THE INSTRUCTIONS OF THE LIFTING SUPERVISOR.
 LIFTING SUPERVISOR WILL HELD A TOOLBOX TALK TO DISCUSE THE ACTIVITY AND EXPLAIN THE ROLES OF EACH PERSON INVOLVED IN THE PLAN.
 LIFTING SUPERVISOR WILL CHECK AND INSPECT THE LIFTING ACCESSORIES AND MOBILE CRANE CHECK LIST AND PAD SIZE AND ALL THE
DOCUMENT.
 SET THE CRANE AS PER THE INSTRUCTIONS OF THE LIFTING PLAN.
 POSITION THE CRANE TO THE C.O.G OF THE LOAD BY BANKSMAN INSTRUCTIONS
 ATTACH THE LOAD TO CRANE HOOK BLOCK BY ACCEESSORIES WITH PROPER RIGGING METHOD AS PER LIFTING PLAN.
 LOAD WILL BE ATTACHED AS PER THE INSTRUCTIONS OF THE LIFTING PLAN AND UNDER THE SUPERVISION OF LIFTING SUPERVISOR.
 Under the instruction of the banksman, the operator will lift the load slowly to get the tension on the slings and check the centerline of the hook
block.
 RIGGERS WILL ATTACH TAG LINES TO THE LOAD
 AFTER CHECKING THE CENTRLINE, LOAD WILL BE LIFTED SLOWELY TO CHECK THE BALANCE OF THE LOAD.
 IF BALANCE IS OK, UNDER THE ISNTRUCTION OF THE BANKSMAN, LOAD WILL BE LIFTED AND ROTATED AND INSTALLED TO THE REQUIRED
POSITION AS PER THE LIFTING PLAN
 ONCE THE LOAD REACHES THE CORRECT POSITION, LOAD WILL BE LOWERED BY SIGNALS FROM THE BANKS MAN.
 ONCE THE LOAD IS INSTALLED, CHECK THE STABILITY OF IT. THEN REMOVE THE ACCESSORIES FROM THE LOAD AND THE CRANE HOOK BLOCK.
 LIFTING SUPERVISOR WILL INSPECT THE ACCESSORIES AND KEEEP IN PROPER SATORAGE.
 MAKE GOOD HOUSEKEEPING FOR THE AREA, REMOVE THE BARRICATION AND LEAVE THE SITE.

The Non-Routine Simple/Complex lifting plan shall be submitted to LTA for review and approval. LTA shall evaluate, in
collaboration with other members of the team, all possible corrective actions and advice to:
STOP! Further engineering inputs and risk assessment are required.

PROCEED WITH CAUTION! Operation may be performed providing that all necessary actions
have been taken and JSA updated accordingly.

GO! Proceed with lifting operation following the instruction given by the PIC.
