    TO WHOM IT MAY CONCERN:

    THIS IS TO CERTIFY that according to the available records of District Mobile Patrol
    Unit, particularly on Police Blotter Entry No.2011-10-1130 dated November 10, 2011 Page 025,
    10:15 am, there appears and entry to read as follows:

    SHOOTING INCIDENT – A telephone call was received by this office from Rey
    Cereneo, 40 yrs. old, married, driver, and resident of 314 Sta. Teresita St., Sampaloc, Manila
    stating that a shooting incident occurred on November 10, 2011 at about 10:00 am in the
    morning At the corner of Dimasalang and A. Maceda Streets, Sampaloc, Manila wherein victim
    one ALEXANDER BAUTISTA y PEREZ, 50 yrs. old, married, driver and lastly resided at 1015
    Craig Street, Sampaloc, Manila was shot several times by suspect one CARLO VILLEGAS y SAN
    PEDRO @ Caloy, 40 yrs. old, married, driver and residing at 1025 Craig Street, Sampaloc,
    Manila who was armed with an unknown caliber firearm and escaped after the incident with the
    weapon used. Further stated that the victim was rushed to Ospital ng Sampaloc, Geronimo
    Sampaloc, Manila but was pronounced dead on arrival. PO2 Jorlan O. Taluban and two (2)
    others were dispatched at the said place of incident to conduct an ocular investigation. After a
    follow up operation conducted by elements of this unit the suspect was apprehended at Del
    Pan, Tondo, Manila. However the weapon used was not recovered.
