    ACCOUNTABILITY

    We, F01 Erwin D Lebios, the outgoing Logistic of established SRU in the
    BFP OPFM Masbate province and F01 Jannery M Aboabo, the incoming
    Logistic, do hereby state and say that;

    We further sayeth that we are already cleared of Property in my possession


    and had been properly turned-over to the incoming logistic, to wit;

    As to Logistics:

    A. Inventory of Rescue Equipment herein folder attached with checked by


    SRU Unit Commander for confirmation.
    B. Copy of Property Acknowledgement Receipt (PAR) issued to SRU
    Personnel.

    To attest to the truthfulness of the foregoing statements, we are voluntarily
