PROGRAM (CSHP)
I, by accomplishing this Construction Safety and Health Program do hereby commit and bind
ourselves to strictly comply with the requirements of the Department Order No. 13, series of 1998 –
Guidelines Governing Occupational Safety and Health in the Construction Industry and the applicable
provisions of the Occupational Safety and Health Standards (OSHS).
I (also) acknowledge our obligation and responsibilities to provide throughout the course of the
project the appropriate and humane working conditions and provision of Personal Protective Equipment
(PPE) as added protection for our workers and employees.
I also (hereby) commit to implement the necessary job safety and health instructions and
training to all our workers for the entire duration of the project, as well as ensure the safety of the
general public.

Company Safety and Health Policy
It is the general policy of TWO ACES CONST. AND SUPPLY to perform work in the safest manner
possible consistent with good construction practices. To fulfill the requirements of this policy, an
organized and effective Construction Safety and Health Program as described in this program and in
consonance with DOLE D.O. No. 13 and the OSH Standards must be carried out on this project.
To achieve these objectives, we are committed to perform the following:
Identifying potential hazards that may arise from the programmed work activities;
Organizing the work activities so as to minimize the risk arising from them;
Developing and implementing project specific safety and health program;
Establishing and enforcing all necessary safe work procedures, rules & regulations;
Ensuring that all workers are given orientation/briefing or induction prior to deployment to the
site;
Establishing a safety and health committee to act as policy making body of the project on all
issues pertaining to safety and health;
Ensuring the conduct of daily toolbox meetings and document each proceeding.
Establishing a system of follow-up of actions and periodic assessment to check program
effectiveness;
Proper documentation and reporting on all issues and concerns on safety and health to
authorized entities.

Duties and Responsibilities of CSHC Committee shall include but are not limited to the
following:
Plans, develops and oversee the accident prevention programs for the construction
project:
Directs the accident prevention efforts of the establishment in accordance with the safety
programs and performance and government regulations in order to prevent accidents
form occurring in the worksite:
Conducts safety meetings at least once a month. The committee will hold a safety
meeting every last Saturday of the month;
Submits reports to the project manager on its meetings and activities one day after the
meeting;
Reviews reports of inspection, accident investigation and implementation of program;
Provides necessary assistance to government inspecting authorities in the proper conduct
of their activities specifically on the enforcement of the provisions of DOLE’s OSHS;
Submit the monthly the following safety and health reports to the DOLE Regional Office
having jurisdiction over the project;
Summary of all safety and health committee meetings agreements;
Summary of all accident investigation reports using DOLE/BWC/OHSD/IP-6 form;
Periodic hazards assessment with the corresponding remedial measures/actions for
each hazard;
Initiates and Supervises safety and health training for employees on a 1 day
workers Safety & Health Orientation.
In case of any dangerous occurrence or major accident resulting to death or permanent total
disability, we will notify the DOLE Regional Office within twenty four (24) hours from
occurrence. After the conduct of investigation by our concerned safety and health officer, we will
report all permanent total disabilities to the DOLE regional Office on or before the 20 th day of the
month following the date of occurrence of accident using the DOLE/BWC/HSD-IP-6 form.
The injured worker must be accompanied by the First Aider to the Nearest Hospital at Province
of _________, for medication.

Toolbox Meeting
D.O. No. 13 define toolbox meeting or gang meeting as a daily meeting among workers and their
respective supervisors for the purpose of instruction, discussion and proper briefing on the
planned work, the assessment of past work, the possibility or actual occurrence of accidents at
the site, tips and suggestions on how to prevent possible accidents and other related matters.
Responsible for the Toolbox Meeting
Our Project Supervisor is required to conduct daily toolbox or similar meeting prior to starting
the tasks for the day to discuss with the workers and anticipate safety health problems related to
every task and the potential solutions to those problems. The supervisor will remind the workers
on the necessary safety precautions that need to be undertaken.
Provisions of PPE shall be in accordance with Rule 1080 of the OSHS (must include Rule 1070
for noise). The equivalent cost for the provision of PPE (life span, depreciation, replacement,
etc.) shall be an integral part of the project cost.
It is important that all PPE be kept clean properly maintained. Cleaning is particularly important
for eye and face protection where dirty or fogged lenses could impair vision. Our workers or
employees inspect, clean and maintain their PPE according to the manufactures’ instructions
before and after each use. Our supervisors are responsible for ensuring that users properly
maintain their PPEs in good condition.
We enforce our rule that PPE’s must not be shared between workers or employees until has been
properly cleaned and sanitized. PPE are disturbed for individual use whenever possible.
If workers or employees provide their own PPE’s, we make that it is adequate for the work place
hazards, and it is maintained in a clean and stored in a conducive condition.
We never allow the use of defective or damaged PPE’s. We immediately discard and replace
them to avoid any unintentional use.
We also consider the importance of ensuring that any contaminated PPE which cannot be
decontaminated is disposed of in manner that protects workers or employees from exposure to
hazard.
We comply with Rule 1412.09 of the DOLE OSH standard in providing a safe covered walkway
over the sidewalk for use pedestrian in a building construction work less than 2.3 meter (7 ft.)
from a side walk or public road.
We comply with Rule 1412.10 of the OSH Standard, Protection from falling Materials. We
observe the following:
We will take the steps to protect worker from falling materials, such as the provisions of
safety helmets and safety shoes.
We will ensure that tools, objects and materials including waste materials will not be
thrown or tipped from a height, but will be properly lowered by hoist or cultures. If such
is not practicable, the area where the material is thrown will be fenced and no person will
be allowed in the fenced area.

Provision of Safety Signages
Our Safety Signages provides warning to workers and employees and the public about the
hazards existing in project site. These will be posted in prominent positions at strategic locations
and visible as far as possible in a language understandable to most of the workers and employees
as well as the public.
The signages include but are not limited to:
Mandatory requirement on the usage of PPE prior to entry to the project site, or type of
PPE to be used when entering a specific site;
Areas where there are potential risks of falling objects;
Areas where there are potential risks of falling;
Areas where there are tripping or slipping hazards;
All places where contact with or proximity to electrical/facility equipment can cause
danger;
All places where workers may come in contact with dangerous moving parts of the
machineries or equipment;
Weekly inspection of such signages to keep it maintained at good and useful condition will be
rendered. Removal and replacement of warning signs will be done as necessary. It will be
headed by ____________________ who is the Safety Officer of the company.

To ensure safe and healthy working conditions throughout the duration of the project the

Safe Work Practices
Safe work practices are procedures adopted for carrying out specific tasks that ensures workers' exposure to
hazardous situations, substances, and physical agents is controlled in a safe manner. Safe work practices are
generally written methods outlining how to perform a task with minimum risk to people, equipment,
materials, environment, and processes.
We adhere to the standards of locating safe work practices where it is readily available to the worker. Some
work methods for safe practice include but is not limited to the following:
Lifting of weights- a male worker shall not be made to lift, carry or move any load over 50kg and
shall be handled by more than one worker or by mechanical means.
Excavation- the walls of every excavation over 1m deep shall be supported by adequate shoring or
timbering to prevent collapse. No excavation in an adjacent building shall be undertaken unless steps
are taken to prevent danger to workers.
Scaffoldings- every scaffolding should be ensured of good construction or of good quality of
material. All scaffolds shall be properly maintained and be regularly inspected for safety. The
construction of scaffold shall be based on what type of scaffold is to be used and ledgers, putlogs,
and standards shall be securely fastened bolts, dogs, or ropes.
Welding and steel fabrications- shall be done by skilled and qualified workers with the use of
appropriate PPEs.
Installation of roofing- shall be done only by skilled workers with the use of safety head gears and
harness.
Installation of CHB for walling- bridging and support shall be well established before engaging to
the installation of walls
Demolition- demolition of scaffoldings shall be under supervision of competent person. No person
except workers who are directly engaged in the demolition shall enter the area within a distance
equal to 1 ½ times the height of the structure being demolished. Danger signs shall be posted around
the structure and demolition shall proceed in a systematic manner
Medical Surveillance
The company will require all employees to undergo a baseline or initial health examination prior to
assigning to a potentially hazardous activity. The examination will include but not limited to the following:
Complete medical and work history;
Physical;
Other special examination;
Random drug testing.
After proper segregation, the wastes will be collected by local garbage officials conforming
on the collection time and place of the local garbage authority.
Construction debris (broken hollow blocks, spoiled concrete, loose concrete, etc) will be
taken out on the staging area and stock-piled at a designated area that will be decided upon by
not only the management but also of the community that is nearby the project site.
Emphasize the importance of good housekeeping by constantly reminding them on
housekeeping rules. Funds from selling waste materials will be collected for use of the
