Guidelines on the Online SLU ID Application for Second- and First-Year


Students for Academic Year 2021-2022

In lieu of the usual in-person SLU ID processing and in compliance with health and
safety protocols during the pandemic, new students of Academic Year 2020-2021 and 2021-
2022 (except for current 2nd Year BSN Students) and old students who enrolled in a new
program during Academic Year 2020-2021 and 2021-2022 (i.e. SOM, SOL, SAS), who are
currently enrolled during this FIRST SEMESTER AY 2021-2022, will be subject to the following
online guidelines and procedures when applying for their SLU Student IDs:

1. Preparation of Individual ID Picture and Signature


A. Picture for ID
1. Prepare One (1) ID Picture (2X2) with at least 640 x 480 resolution with royal
blue background.

2. Wear appropriate attire (with collar). Sandos or collarless shirt for the
gentlemen and sleeveless clothes for the ladies will not be allowed.Students
are to remove unnecessary accessories, e.g., piercings, headbands, necklace,
caps, etc.
3. The student’s face must be clear and distinguishable and taken at an eye-level
position. Please do not use camera filters.
4. Ears must be visible in the ID Picture.
5. Students are to sit up straight and look straight at the camera and shoulders
must be visibly straight in the ID Picture.
6. The picture must be saved in .JPEG or .JPG format. The ID Picture must be saved
using this format: Student ID FamilyName, FirstName Pic (i.e. 222111 De la Cruz,
Juan Miguel Pic).

B. Signature Taking for ID


1. Signature picture must be at least 480 X 320 resolution.
2. Signature must be written in a clear white bond paper using BLACK BALL PEN
only, preferably using a sign/gel pen. Students are to take a photo of or scan
the paper for the signature specimen.
3. Signature must be distinguishable with no superimpositions.
4. The signature must be saved in .JPEG or .JPG format. The signature must be
saved using this format:Student ID FamilyName, FirstName Sig (i.e. 222111 De la
Cruz, Juan Miguel Sig).

2. Submit Personal Information, ID Picture and Signature using the QR Code below.
1. Please scan the QR Code below.
2. Answer completely, properly, and truthfully the required entries as these will be
the basis of the information that will appear on your SLU Student ID.
3. For erroneous entry encoded by the student, the student will be charged an ID
Duplication Fee when requesting for a new ID.

3. Claiming and Release of SLU IDs


1. As we will be expecting a volume of IDs to be prepared and printed, please expect
your IDs to be available within a month after your submission of the requirements.
2. IDs will be released on campus to ensure that the details of your IDs are personally
checked by you before it will be released.
3. Claiming the IDs will be by APPOINTMENT ONLY and your entry into the university
will be subjectto safety, health, and triage protocols. You will need to text
(0908)284-4467 for your appointment and wait for a text confirmation of your
schedule before you may come to SLU. Please take note that without a
confirmedappointment, you will NOT be entertained. If you fail to come on your
scheduled appointment, then you will have to request for a new appointment.
4. If you cannot personally come on the release date of your Student ID, you may
send an Authorized Representative to claim it on your behalf. Please wait for the
confirmation of the release of your ID beforesending your authorized
representative. An Authorization Letter Template is available on the downloadable
forms of your iSLUportal for this purpose.

SLU Institutional email when inquiring. Non-SLU emails will not be entertained.
