We thank you for your interest in Inspire Ventures and our logistics startup Transportify. Prior
to your interview with us, please complete the below questionnaire and return it via email to
the names listed at the bottom of this form.

● Please estimate the time of your commute to work. Our office is located at
Tycoon Center Building, 23rd Floor, Pearl Drive, Ortigas Center. Due to the
ongoing pandemic, our employees are temporarily working from home but once
the situation stabilizes, we are going to require everybody to work in our Ortigas
office.

● Why do you wish to leave your current job? Or why did you leave your most
recent job? Please briefly explain. Put N/A if you are a fresh graduate or if you do
NOT have any prior work experience.

-I finished our project based account from October to December. My plan was to
accept the job for 2 months then I will be spending the holiday season with my
family. I already planned to look for a job after the celebration
of the new year. Just to inform you about my last work experience, it
was a project based account for 2 months. The company tried to absorb
our team of experts and gave us ample time to re-sign the contract and
offered a new position in the company to join the Universal Service
team. However, they're offering us a much lower salary compared to
what we are receiving during our project based stay. I decided to
decline the offer since I'm looking for better compensation to support
my family needs. I hope you understand my decision as the breadwinner of the
family.

● What is your expected salary? Be realistic.

- 30,000PHP

● Are you paid additional bonuses or commissions? If yes, please describe. Please
clarify if the amounts are before or after tax withholdings.

-Non-taxable allowances for 2,000php plus Skills allowance of 6,000php

● What other benefits do you receive? Examples include healthcare, allowances &
healthcare insurance

-Healthcare insurance and Life plan

● For all the amounts listed in questions above, if we request it, would you be able
to provide payslips, bank statements, or other official evidence to our HR
department?

-Yes, I can provide for the contract. We cannot generate payslips since we are
only using company provided email for us to receive/ view our paylips.
● If you were to accept an offer of full-time employment, what is your earliest start
date?

- March 31 Friday

● Have you ever had either directly or through friends/family exposure to a start-up
company before? If yes, please briefly explain
