DARP Charting

A comatose and drowsy Male client Mr. Ulcer 88 years old, admitted in unit # 95, R# 03 and bed
# 02. HCA is taking care of the wound of the client. Client has bed sore on left buttock with
bone involved. During cleaning HCA noticed blackish streaks on hip bone with foul smell and
client is not responding to vocal command of HCA. During cleaning of wound client does not
feel sensation on wound. HCA checked VS and P.77, BP. 160/ 110 mmhg, RR. 18, O2 sat 95%.
HCA read MPR and found PRN medicine Tylenol 500 mg 6H for 6 days. HCA finished wound
dressing and checked VS found normal and gave pain medication PRN.
