    who died last July 11, 2022 insured under LOAN PAYMENT PROTECTION INSURANCE (LPPI) Plan
    requesting for the following:

    1. Why there is no loan payment?


    2. Correct Certified Loan Ledger

    Please be informed that 1CISP has not received any response on your end. Because of this, no
    claim decision has been made on this claim application. If you wish to continue processing of
    your claim, kindly submit an Affidavit explaining the reason of your late compliance and the
    above-mentioned required document.

    If we do not receive a response from you within seven (7) days from receipt of this letter, we
    will deem your claim cancelled. Compliance can be sent through Claims Department’s official
    e-mail address, cispclaims@gmail.com.
