AFFIDAVIT OF CHANGE OF NAME AND CIVIL STATUS
I, JESSICA O. ARCABAL , Filipino, 29 years old and a resident of Brgy. Mercedes,
Catbalogan City, Samar, Philippines, after having been duly sworn in accordance with law do
hereby depose and state;

1. That I was born as JESSICA CABAHUG OCHEA on March 31,


1991 at Catbalogan City, Samar as shown in my Philippine NSO /PSA
Birth Certificate;

2. That my civil status appearing in my records with the Samar State


University is SINGLE using my maiden name JESSICA CABAHUG
OCHEA;

3. That I am now presently married having been joined as husband and


wife together with ALBERT ALIGADO ARCABAL, of 29 years old,
and a resident of Brgy. Mercedes Catbalogan City, Samar in
matrimonial celebration on 5th day of March, 2019 at Assembly of
God Church Catbalogan City, Samar and the fact of our marriage as
reflected in our Philippine NSO/PSA Marriage Certificate;

4. That I intend to change my name from JESSICA CABAHUG OCHEA,


which I had been using in Samar State University to JESSICA
OCHEA ARCABAL to reflect my husband’s surname and signify
married status;

That I am executing this affidavit to attest to the truth of all the


foregoing statements and for whatever legal purpose it may serve.

RE: CHANGE OF NAME AND CIVIL STATUS IN SCHOOL RECORDS

We, VILMA M. ABADIANO and JULIUS D. ASILO both of legal age, Filipino, and
both residents of Brgy. Mercedes, Catbalogan City, Samar, after having been duly sworn in
accordance with law do hereby depose and say;
1. That we personally know JESSICA CABAHUG OCHEA , whose same name
appears in the records of Samar State University;

2. That we know based on our personal knowledge that JESSICA CABAHUG


OCHEA got married to ALBERT ALIGADO ARCABAL on the 5th day of
March, 2019 at the Assembly of God Church, Catbalogan City, Samar;

3. That JESSICA CABAHUG OCHEA’ name should be changed to JESSICA


OCHEA ARCABAL and her status from SINGLE to MARRIED from the
records of Samar State University;

4. That we personally know the foregoing facts because we were present during the
marriage ceremony and we are friends and neighbors with JESSICA OCHEA
ARCABAL for 20 years up to date;

5. That we are not related to spouses JESSICA OCHEA ARCABAL and


ALBERT ALIGADO ARCABAL either by affinity or consanguinity and has no
interest in whatever this affidavit could be use directly or indirectly;

6. That we execute this JOINT AFIDAVIT to attest to the truthfulness of all the
foregoing statements and to support the application for CHANGE OF NAME
AND CIVIL STATUS IN SCHOOL RECORDS of JESSICA OCHEA
ARCABAL, and for whatever legal purpose this affidavit may serve.
