    INSTRUCTIONS
    STUDENT: Take this form to your School Director. Ask the Director to complete the form, sign it, and stamp it with an official school stamp. Give the grading
    scale for each academic year (for example, either 1-5, 1-10, 1-12, or 1-100). Use numbers to show marks. If a subject was not taken, leave space blank. You
    must return this document as part of your completed application by the due date shown on FORM 1. Finalists will be asked to provide the program with
    original transcripts, issued by their home schools, for the past 2 years.
    SCHOOL DIRECTOR: If the student is excused from any class(es), or has any educational accommodations, please note the reason next to the class (for
    example: “excused from math school due to a math-related learning disability”).
