Checking Procedure for SGOD
BASED FROM THE IDENTIFIED SECTIONS TO BE CHECKED FROM THE DRAW LOTS
1. Ask the SCC to present the LIS-GENERATED SF1 of the identified section. Verify if it is LIS-generated by looking
at the bottom LEFT PART of the form with the remarks “Generated on ____________________”
2. From the SF1, choose 1 or 2 or 3 learners.
Say learner Juan Dela Cruz, ask for the SF10 of that learner.
Check if the LRN, Name and Birthdate and Gender in the SF1 matches the entries in the SF10 and is consistent
with Birth Certificate or its equivalent docs. If not matched, advise school to adjust SF1 thorough
LIS Learner Profile.
3. Proceed to the next learner you want to check.
4. Now, ask for the SF4. Please check the NUMBER OF CURRENTLY ENROLLED in SF4 with the NO OF LEARNERS in
SF5. If mismatched, validate list of learners in SF5 vs SF2 for May or June 2023. Only the currently enrolled
learners must be reflected in SF5.
5. Now, compare SF6 and SF4. The number of learners in that selected section or class must tally in count.
6. Record any findings using SFCR1.
SF1 (LIS-generated) and prepared by ADVISER. SF4 is manually prepared by SCHOOL HEAD.
SF2 (manually prepared) by ADVISER. SF5 is LIS-generated and prepared by ADVISER.
SF1O is prepared by ADVISER. SF6 is manually prepared by SCHOOL HEAD
1. SF1 is School Register 4. SF5 is Report on Promotion and Level of Proficiency
2. SF2 is Daily Attendance Report of Learners 5. SF6 is Summarized Report on Promotion and Learning Progress and Achievement
3. SF4 is Monthly Learner Movement and Attendance 6. SF10 is Learner Permanent Record
Checking Procedure for CID
BASED FROM THE IDENTIFIED SECTIONS TO BE CHECKED FROM THE DRAW LOTS
1. Ask the SCC to present the SF5 of the identified SECTION or CLASS.
3. From the SF 5, you choose one or two or 3 learners or more,
4. Ask for the SF10 of the said learners.
Compare and check the following:
>> The promotional status (Promoted, Conditionally Promoted or Retained) must be reflected in the SF5
and beside the GENERAL AVERAGE in the SF10.
>> Check for the SF10, ensure that all entries are complete with remarks or action taken.
>> Observe failed subjects that they should be given remedial classes.
5. Check also SF10 if it contains all attachments or supporting documents needed.
For learners who dropped out during the SY, their SF10 must indicate across the covered SY, the following
remarks: No Longer Participating in Learning Activities (NLPA) due to ___(reason)___ effective __(date)__.
