Tuberculosis burden
In 2019, an estimated 599,000 (range: 336,000-936,000) people fell ill with tuberculosis in the
Philippines, of which 373,000 (62%) were men, 157,000 (26%) women and 69,000 (12%) children.
An estimated 27,000 (range 23,000-31,000) people died from TB in 2019. Between 2015 and 2019
TB mortality in the country has been reduced in average 2.8% annually.
In 2020, 257,000 new and relapse TB cases were detected by health systems in the Philippines.
This reflects a gap of 342,000 (57%) between estimated and notified cases.
Disruption of the services in 2020 due to the COVID-19 epidemic reversed the gains of decades. In
2020 about 152,000 fewer people with TB were detected by health systems in The Philippines
compared to 2019.

Screening
STRATEGIC INTERVENTIONS
Intensified case finding (ICF): systematic screening in health facilities by offering symptom
screening, high-risk screening and chest X-ray screening among all adult consults.
Active case finding (ACF): systematic screening in high-risk communities (for example, urban
and rural poor), workplaces or congregate settings (for example, jails and prisons) using
symptom screening, and mobile chest X-ray screening followed by rapid molecular testing.
Enhanced case finding (ECF) community-based symptom screening and provision of sputum
collection and transport services (for example, using Specimen Transport Riders for STRiders).
Contact investigation: Integrated contact investigation (and LTBI case finding) as part of ICF,
ACF and ECF.
In 2020 in total 63,240 prison inmates were actively screened for TB according to national guidelines.
This is remarkably lower then to 144,770 prisoners screened in 2019 and only 30% of the total estimated
215,000 prison population in the Philippines. Among those screened 7,904 (12.5%) were identified with
abnormalities by chest radiography. 8,019 were tested by Xpert MTB/RIF.
In total 1,977 were identified with TB among the prison population. This is 42% lower compared to 3,414
notified in 2019. Among those, enrolled only 720 (36%) were bacteriologically confirmed compared to 979
detected in 2019 (Figure 2a).
Number needed to screen (NNS) to identify one bacteriologically confirmed TB case was only 88 in 2020.
The chest Xray of 12.5% of inmates screened in 2020 were read as suspicious for TB, ranging from 2.8%
(Region X) 26% in (Region XI) across the regions. For comparison, during TB prevalence survey in the
Philippines 29% of adult population screened by chest Xray were classified suggestive for TB.
Xpert MTB/RIF positivity was 9.0% at the national level among those tested, with excessive rate of
positivity in Region X (66.6%), Region VIII (35.4%), Region XII (23.4%) and Region XI (20.7%).

- Screening coverage among prison population is insufficient and remarkably declined in 2020
compared to previous year.
- The proportion of patients with chest Xray read as suggestive for TB also is much lower than
expected, indicating that most likely some patients with TB are missed at screening stage due to
low sensitivity of the methods applied, including sensitivity thresholds for the CAD-AI reading.
- Overall, the yield of TB among screened is lower than expected: globally, the estimated incidence
of TB among people residing in prisons is 23 times higher than that among the general
population1. In 2020, the yield of BCTB among screened inmates was 1,138 per 100,000
compared to 599 per 100,000 estimated incidence in general population and 2,100 per 100,000 TB
prevalence among inmates in a 2015 study (PTSI).
- Data related to prison TB are not comprehensive, as they include only specific programmatic
interventions (PBSP) while the routine case-finding from the jails that have their own DOTS, are
not included in the report of TB notification from the penitentiary system, therefore data might be
under-estimated.
- Main reason of the drop of ACF activities is because of CIVID 19 situation ACF resumed only on the
4th quarter of 2020 but didn’t reach pre-pandemic level. Prison authorities, especially from the
large prisons, are still reluctant to conduct ACF activities. Strict measures are undertaken and
there are difficulties to organize ACF in prisons.
Recommended actions
- Advocate Department of Interior and local Government (DILG) to release memorandum allowing
ACF in prisons, considering minimum public health standards. ACF has been resumed in small
prisons, but possibilities still are limited.
- Evaluate diagnostic accuracy (sensitivity, specificity) current threshold score of computer aided
diagnostics (CAD) employed in the prison settings of Philippines and calibrate the threshold of the
software accordingly to ensure that the accuracy, predictive values, overall yield and requirements
for further diagnostic testing are as expected.
- Improve the recording and reporting of prison TB cases, including those detected outside the
programmatic interventions and by prisons that have their own DOTS structures.
- Consider to assign volunteers or health officers to collect sputum from jails similar to COVID 19
ACF practice.
- Introduce, promote recording and reporting of screening activities carried out among general
population.

0.54% of population in the Philippines in 2020 were tested either by Xpert MTB/RIF (0.37%) or direct
sputum smear microscopy (0.17%). This is far below the target of 2.3% population testing coverage. The
highest testing coverage was reported in Region XIII, Region II and Region V (Figure 2).

Recommended/planned actions
- There is a lack of information related to how accessible is bacteriological testing for the patients.
One of the indicators of accessibility of the test could be turnaround time. NTP should establish a
system to monitor the turnaround time, which may to help to understand if there are hassles to
patients
- NTP currently is introducing software to monitor remotely the down-time of each Xpert MTB/RIF
machine.
- All operational sites currently are supplied with PPE and number of tests during the first quarter of
2021 already reached to 2020 Q1 level.
- Improve access of private sector to Xpert testing.

63% of notified new and relapse TB patients were tested using Xpert MTB/RIF. This is remarkably higher
compared to 37.4% in 2019, exceeding the national target of at least 60% RDT testing coverage in 2020.
Eleven of 17 regions over-achieved the target. The highest Xpert MTB/RIF testing coverage was reported
in Region VI, Region VII, and Region XIII (Figure 3).

vertical line depicts the target of this indicator for 2020. In 2020, Xpert testing coverage ranged from
42.9% in Region II to 78.9% in Region VI. In all regions, without exception, the Xpert MTB/RIF testing
coverage increased.
This impressive progress is explained by the policy revision. Starting from 2020, Xpert MTB/RIF became
the primary diagnostic test, while until 2019, the rapid tests were limited to children, people living with
HIV, RR-TB contacts, and those with previous history of treatment. However, it is believed that Xpert
MTB/RIF testing coverage among new and relapse TB patients is under-estimated due to incompleteness
of the coding of Xpert test result in ITIS. There are no automated connectivity with GeneXperts and it is
required manually to re-enter GeneXpert outputs. Apart the increased workload such approach might lead
to errors in data transmission. Among 37% of new and relapse TB patients without Xpert results 12.7% are
those contributed by mandatory notification.

- NTP currently is planning to introduce software to monitor remotely the down-time of each

- Additional Xpert/Truenat machines to be installed to further improve access.

Coverage of second-line drug-sensitivity testing (DST) among all patients with RR-TB according to ITIS was
13.8% down compared to 20.7% SLD coverage reported in 2019 (Figure 4).

Low coverage of second-line DST is explained by incompleteness of coding of LPA results into ITIS.
According to NRL in 2020 nearly 4,500 SL LPA test results have been conducted and about 95% of RR-TB
patients had documented FQ DST results.
Recommended/planned actions
- NRL dedicated a separate staff to ensure that LPA test results are coded in IT IS. Data will be
completed retrospectively
- NTP will promote the use of electronic lab request and result form, to improve automatic
reporting of LPA result in DRTB register.

13% (32,808/256,848) of notified new and relapse cases in 2020 were contributed by mandatory
notification. Two regions (Calabarzon and Region III) exceeded the target of at least 20% being notified
through mandatory notification, followed by NCR (Figure 5). Achievement of mandatory notification
indicator in Big3 is explained by impact of Global Fund supported project, which hired notification officers.

In 2020 in total 45,926 new and relapse TB patients were notified by MN (vs. 73,145 in 2019). Notified
number of MN was further reduced to 32,808 after removed of repeat observations in ITIS, resulting the
decline of contribution of mandatory notification. Before the removal of repeat observations from the
database, relative contribution of MN was 16.7%, which is comparable to 2019 results. However, sharp
decline of absolute number of MN cases is explained by COVID-19 situation. Most of the private clinics
were closed during the onset of the pandemic. Up to now, most still operates on limited schedule and
limits also the number of patients for consultation. Outpatient department were closed, and limited
consultations were being done.
Although at the national level NTP targets that 20% notification is contributed by MN, however, due to
large variability of the size of private sectors across the regions and provinces, it is not expected that the
target will be feasible to achieve by all sub-national area. Thus, patient pathway analysis (PPA) conducted
in 2017 indicated that at national level 35% of patients initiate care in the private sector2, while at the
regional level this indicator ranged from 44% in Region III to 22% in Region IX (Figure 6).

Recommended/planned actions
- Under the GF grant, NTP have started scaling-up support for MN to cover other highly urbanized
cities outside of the big 3 regions. Between 2018-2020 support was only for the big 3 regions,
which resulted notable increased of TB notifications from private sector.
- NTP also moving towards electronic laboratory request and results forms that will automate Xpert
result scale-up in other regions outside GF support is also spearheaded by the DOH regional
offices
- Notification at point of testing (i.e., by bacteriologic testing laboratories) will be implemented to
ensure all BCTB cases are notified
Due to COVID 19 pandemic, in 2020 about 152 thousand fewer people with TB were diagnosed in the
Philippines compared to 2019, resulting in a 38% decline in TB notification rate at national level. The
sharpest decline was observed among the clinically diagnosed cases. The decline of bacteriologically
confirmed cases between 2019 and 2020 was about 21%. (Figure 7).

Increased proportion of bacteriologically confirmed cases as well as sharp decline in number of clinically
diagnosed TB patients is partially explained by increased coverage of Xpert MTB/RIF testing which is more
sensitive, compared to DSSM.
42% of notified new and relapse TB patients were bacteriologically confirmed compared to 33% in 2019.
Only five of 17 Regions did not reach 2020 target of least 40% bacteriological confirmation among new
and relapse TB patients. The top 3 regions with highest proportion of bacteriologically confirmed cases
were Region XI, Region VIII and Region V. The highest notification rates bacteriologically confirmed TB
cases were observed in Region VII, Region XI and Region XIII.
The Mandatory Notification from the private sector are almost all clinically diagnosed TB cases.

After the start of the COVID-19 pandemic from the second quarter of 2020, due to disrupted health care
seeking, and imposed lockdown in the Philippines, the quarterly number of Xpert tests and case detection
at the country level fell by more than 75% between March and April. Subsequently, there has been some
recovery, but as of the end of 2020, in all regions the testing has not returned to pre-pandemic levels
(Figure 9).
Turnaround time from specimen collection to release of Xpert results among 44,527 new and relapse TB
patients with available data in ITIS was only 0 [IQR; 0-1] day comparable to 2019 result and no variation at
the regional level (Table 1). However, considering that STRiders are transporting a significant proportion of
specimens, which is likely to be causing delays between specimen collection and laboratory reports.
Hence, zero median value between specimen collection and lab reporting may be mostly because the data
are incomplete and/or incorrect.

As indicated above, decline in notification, a large proportion of reported cases comes from the private
sector (about 39% in 2019 in the big three regions). In 2020, total cases reported declined for mandatory
notification by more than 50% in the same regions. Mandatory notification numbers were low because
hospitals were mainly affected. There was also a demand-side problem: From one of recents studies
conducted in in Metro Manila "Among those who needed to seek help or treatment since quarantine
began, 66 percent (~8 percent of all families in study sample) did not receive sufficient help or treatment.
The main barriers were transportation inconvenience (24 percent) and limited availability of medical
professionals (15 percent)."

Treatment
STRATEGIC INTERVENTIONS
Establishment of Health Care Provider Networks (HCPN) offering the full TB care continuum.
256,848 new and relapse TB cases were notified by health care providers, equivalent to 236 per 100,000
population. This is only 43% of the 599,000 estimated TB cases in 2019. Also, this is 38% lower compared
to 409,167 TB patients notified in the previous year.
6,411 RR-TB patients were identified, which is 30% of the 21,000 estimated RR-TB cases in the
Philippines in 2019. This is 14% lower compared to 7,492 patients notified in the previous year.
71.8% of new and relapse TB patients notified in 2019 were successfully treated. When the analysis was
restricted to those with reported treatment outcome the treatment success rate achieved 92%.
67.4% of RR-TB patients enrolled for DR-TB treatment in 2018 were successfully treated compared to
58% for the 2017 cohort. 76% of detected RR-TB patients were enrolled into second-line TB treatment.
33.4% of notified TB patients had known HIV status in 2020, comparable to 2019 data.

Magnitude of decline in TB notification in 2020 compared to 2019 was negatively associated with
incidence of COVID 19 (Figure 11).

Ecological analysis of TB notification over the poverty showed negative correlation between TB
notification and poverty incidence at province level. TB detection is highest in provinces with lower
poverty incidence and TB notification decreases with increase of poverty. While this correlation
disappeared when the analysis was limited to new & relapse bacteriologically confirmed patients (Figure
12)

The lack of correlation in bacteriologically confirmed new and relapse TB notification indicates that the
access to bacteriologic test is almost the same regardless of poverty incidence. But negative correlation in
total NR notification might mean access to services for clinical diagnosis (e.g., CxR services, TST for
children) is more difficult in poor settings.

As of end of February, treatment success rate of new and relapse TB patients was 71.8%. About 23% of
patients were not evaluated for the treatment outcome. Region XII, Region X, Region XIII and Region II
already achieved at least 90% treatment success rate.
Nearly 99% of MN patients in 2018 were not evaluated for the treatment outcome, while among the
patients that received care at DOTS facilities, the proportion of not evaluated was 2% only.
Recommended/planned actions
- Because the vast majority of cases with notified by MN are not evaluated, it is essential to
promote the evaluation and reporting of treatment outcome among private providers. For this
purpose NTP intends to increase use of ITIS among private providers with the added feature
sending alerts to report outcomes after 6 months. This needs advocacy from patients and
professional societies.

Nine of 17 Regions achieved at least 71% treatment success rate among RR/MDR-TB patients enrolled into
treatment in 2018. The highest treatment success rate among RR-TB patients was achieved in Region I
(81.0%) followed by Region VIII (79.9%) and Region II (78.6%).

To ensure effective integrated TB and HIV service delivery, WHO recommends provision of HIV testing to
all TB patients, providing ART and CPT to TB patients living with HIV. Although the national level HIV
testing coverage is close to the targeted 35% HIV testing coverage, however, this is a result of
disproportionate testing practice across the regions, indicating that most likely some people with TB/HIV
co-infection are missed and to not get appropriate integrated TB/HIV care.

In the figure above, light red circles and dark red circles indicate percent of new and relapse patients with documented
HIV test results corresponding to 2019 and 2020 at the regional and country level and the dotted vertical line denotes
the target of this indicator for 2020. In 2020, HIV testing coverage ranged from 0.6% in ARMM II to 67.1% in NCR.
Overall, the testing coverage remained stable across the regions: in Region XII, Region III and NCR HIV testing coverage
slightly increased, while in Mimaropa and CAR testing coverage slightly declined. At the national level the indicator is
stable and just slightly below of PhilSTEP 2020 target of 35%.
Analysis of case-fatality of new and relapse patients by HIV status, at national and most of province level
indicates that patients with unknown HIV status show higher case-fatality compared to those with negative
HIV status, probably due to undetected HIV status (Figure 17)
