    If a truck breaks down in the WCX3A tunnel, the following procedure should be followed: 1. Report the incident to WCX and Holcim and fill out incident reports, taking photos of the truck and site conditions. 2. Contact Holcim to report the incident and have them call GRS Towing if the truck cannot be driven safely. 3. Once back at the yard, fill out another incident report, discuss saving footage if the incident was recorded, and get the truck fixed by submitting documentation like incident reports and invoices.

    Truck Breakdown in WCX3A Tunnel

    If the truck breaks down/tyre is punctured, Following procedure to be


    followed:

    Step 1:

    . Report the incident to WCX and fill out an incident report. Take a photo/keep
    a
    copy for your record, Holcim will request this. Take photos of the truck and
    site conditions, if this was the cause of the truck breakdown or damage.

    Step 2:

    . Make sure you contact Holcim central allocation/batch office as soon as


    possible
    to report the incident. If the truck can not be driven safely, Holcim will
    support you and call GRS TOWING to get the truck towed out of the job
    site. Holcim has used GRS TOWING in the WCX tunnel and WCX have
    authorized the use of this tow company.

    Once you come back to Lidcombe yard, report to the batch office, fill out the Holcim incident
    report. Please discuss if truck footage needs to be saved if the incident would be
    recorded in footage.

    Get your truck fixed and submit the following


    documents:

    Site filled and submitted incident report copy -


    WCX. Photos of truck/tyre damage
    Photos of site conditions if it aided to the damage
    • Holcim incident
    report
    Camera footage if required Receipt/invoice of
    the to rectify the damage

    Please note, all the above are required for any project claim, failure to provide the
    above may result in the claim being rejected.
