The second part will be the Certificate of Assent. In the last paragraph of the letter, add the contact number of UBREC. Part 3 is the Affirmation by the Researcher B. Consent form for the parents should also be in 3 parts. First part is the Consent form Accomplished letter to the parent. Page 2 Second part is Certificate of Consent which should state “I allow my child to be a participant of the study.” The Visayan translation is okay. Third part is the researcher’s affirmation.
