2. You will use the EHR Go queries tool to complete this activity. Your instructor has
provided you with a link to the Query - Readmission Analysis (AS) activity. Click on 2:
Launch EHR to launch the queries tool and begin this activity.
3. You’ve been granted access to certain data in the EHR to complete this activity. The
approved data has already been linked to the queries tool in the EHR.
4. Document your answers directly on this activity document as you complete the activity.
When you are finished, you will save this activity document to your device and upload
this activity document with your answers to your Learning Management System (LMS).

The activity
Reducing unnecessary hospital admissions and readmissions is a key component of healthcare
quality improvement (Forum, 2017). A readmission is classified as an unintended readmission
to the hospital within thirty (30) days of discharge. Certain diagnoses are known to have higher
readmission rates and can be used to assess the overall quality of the healthcare provided
during an admission.

The General Hospital is doing a self-assessment of readmissions and would like you to prepare a
report with data on the following selected discharge diagnoses: gangrene, heart failure,
unspecified kidney failure, and sickle cell disorders. Compare the findings to the readmission
rate across all other admissions.

Access the EHR queries tool and corresponding dataset under 2: Launch EHR. Click the New
Session button to launch the EHR.

Generate various queries to answer the questions in the next section. An example query for
one of the diagnoses (gangrene) is provided below. Hint: Just repeat the same steps for the
additional diagnoses.

Gangrene Analysis
On the Queries tab, select New to initiate a new query.

Enter the following then Query.


Name: Gangrene discharges
Query Rules: ‘Discharge Diagnosis’ equal ‘I96 Gangrene’

After running the query, select Close. Repeat the steps for the additional new queries for the
gangrene discharge diagnosis.

Name: Gangrene not readmitted


Query Rules: ‘Discharge Diagnosis’ equal ‘I96 Gangrene’
AND (select +Add rule)
‘Readmission Status’ equal ‘Not readmitted <30 days’

Repeat these same queries for the additional discharge diagnoses (heart failure, unspecified
kidney failure, sickle cell disorders, and all other admissions). Once finished, go to the Queries
tab summary view, which you will see after closing out of the last query you run, and note the
Total Record(s) for each resulting query. Use those numbers to answer the questions below.
