    AFFIDAVIT OF UNDERTAKING

    I, ___________ _____, of legal age, Filipino, single/married, a resident of __ __


    ___ , representing ____________________ having sworn in accordance with law, do hereby undertake to
    abide by the General Guidelines under the Presyong Risonable Dapat (PRD) Program of the Department of
    Trade and Industry on Direct Importation of Rice by Retailers and Importers, and hereby state that:

    1. I am the authorized representative of our company to execute this Affidavit of Undertaking, as evidenced
    by Board Resolution No. _____ issued on _______ and attached hereto as Annex “A”;
    2. I specifically undertake in behalf of our company that the Rice and/or Sugar that we will import under the
    Presyong Risonable Dapat! Program of the Department of Trade and Industry shall be strictly sold at
    Php38.00 and/or P50.00 per kilo in all our retail outlets, and violating such shall be a ground for the
    automatic cancellation/revocation of the NFA License to sell and import, and/or filing of appropriate
    criminal, civil and/or administrative action against me and/or the entity I represent;
    3. My company commits to submit a Monthly Inventory and Sales Report to the NFA and DTI and such
    other reports as may be necessary or required by the said government agencies;

    4. Importation must pass the physical and quality pre-shipment test by accredited and certified 3 party
    inspection bodies;
    5. My company shall comply with the mandatory labelling of the PRESYONG RISONABLE DAPAT!
    Program, to include the name of the retailer and the country of origin, with markings “To Be Sold at Not
    More than P38.00/kilo Retail” for Rice and “To Be Sold at P50.00/kilo for Sugar;
    6. All information supplied in the Company’s application are true and correct to the best of my knowledge;
    7. Any false or misleading information supplied, or production of false or misleading document to support
    this application shall be a ground for the automatic denial of this application, automatic
    cancellation/revocation of the NFA License to sell and import, and/or filing of appropriate criminal, civil
    and/or administrative action against me and/or the company I represent;
    8. I fully understand and hereby agree without any reservation that my failure to comply with or observe
    any of the foregoing undertakings or any of pertinent rules and regulations shall be sufficient ground for
    the denial of my application or cancellation/revocation of my NFA License to sell and import rice.
