    AFFIDAVIT OF LOSS

    I, ____________________________, Filipino, of legal age, married, and with residence


    and postal address at Kabacsanan, Iligan City, Lanao del Norte, after having been duly
    sworn to in accordance with law, hereby depose and state, THAT:

    1. I was subsequently issued a CERTIFICATE OF REGISTRATION (C.R.) bearing


    no. 176514763, corresponding to a motor vehicle particularly described as
    follows:

    MAKE : KAWASAKI MV FILE NO. : 0930-00000057464


    CHASSIS NO. :
    TYPE OF BODY: MOTORCYCLE
    MH4LX150DDKPO2375
    ENGINE NO. : LX150CEP88547 PLATE NO. : JA 19742

    2. However, said Certificate of Registration (C.R.) of the above-described vehicle


    was lost, and its whereabouts cannot be ascertained despite due and diligent search
    made and undertaken, such that I now believe that the same was lost beyond
    recovery;

    3. In view thereof, I now request Land Transportation Office (LTO) to issue another
    Certificate of Registration (C.R.) in lieu thereof;

    4. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing
    and for whatever legal purpose this affidavit may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 25 th day of October 2019,
    in Molave, Zamboanga del Sur, Philippines.
