β-blockers are strongly recommended for patients suffering from
cardiovascular diseases, particularly in HF because of their impact on

mortality reduction. They. act by blocking β receptors that are expressed
mainly in myocardial tissue, leading to a reduction in heart rate and
myocardial contraction and therefore to a lower oxygen consumption
COPD patients, an ICS/LABA combined with a LAMA is the most

prescribed treatment strategy even in the least severe stages of disease
regardless of the presence of HF comorbidity.
In 88% of cases, the β-blockers used were cardioselective. The authors

did not find any association between the use of β-blockers and FEV1
decline in patients treated with LABA or LAMA.
Pharmacological treatment for HF that influences COPD patient

prognosis
Cardioselectivity is a priority, metoprolol, bisoprolol, and nebivolo are

the best candidates.
The benefits of selective β1-blocker treatment in HF clearly outweigh any
potential risk associated with treatment in COPD patients, even in those
with severe obstruction
. Low dose initially and gradual up-titration is recommended. Mild
deterioration in pulmonary function or presence of mild respiratory
symptoms should not prompt discontinuation. Among patients with HF,
continuing β1-selective β-blockers during hospitalization for COPD
appears to be safe. Nonselective beta blockers

Beta blockers (especially nonselective beta blockers) can worsen pulmonary symptoms of
asthma and COPD by causing bronchoconstriction. However, in patients with concomitant cardiac
disease beta-blocker use is beneficial in improving cardiac symptoms and decreasing mortality.
Cardioselective blockers are recommended in this group of patients.
Treatment with statins, angiotensin-converting enzyme (ACE) inhibitors,

and angiotensin-receptor blockers (ARBs) can reduce the morbidity and
mortality of COPD patients.
In patients with absolute contraindications for β-blockers, the
association of ACE inhibitors and ARBs could be a treatment option for
increasing survival.

chronic hypertensive disease
In vivo and in vitro studies have found that angiotensin II (AngII)

stimulates the release of inflammatory cytokines such as interleukin,
tumour necrosis factor α, and monocyte chemotactic protein 1.
potentially driving the local and systemic inflammatory response in
patients with COPD. AngII also increases inflammatory cell migration,
epithelial cell apoptosis, and fibroblast activity in the lung
parenchyma.78,79 It enhances bronchoconstriction driven by muscarinic
receptor activation,80 and drives the cytokine mediated immune response
to lung injury. ACEIs or ARBs can improve histological appearances, lung
compliance, exercise capacity, and reduce lung fibrosis in animal models
of pulmonary disease
The underlying mechanisms of benefits of SGLT2i are much debated but
are likely to be due, mostly, to the osmotic diuretic effect of
glycosuria.120 Thus, in those with HF, SGLT2i might decrease pulmonary
congestion,121 and theoretically improve lung function, reducing the risk
of infective COPD exacerbations
high doses of loop diuretics can produce metabolic alkalosis, with the
presence of hypoventilation as a compensatory mechanism, which in
turn could worsen hypercapnia
Loop diuretics are vital for controlling symptoms of venous congestion in

HF.50 Venous congestion is also a feature of cor pulmonale, and thus loop
diuretics are prescribed to some patients with COPD with no diagnosis of
left ventricular dysfunction. Pulmonary congestion may cause worsening
of lung function and obstructive physiology on spirometry.36 Titration of
HF medications (including diuretics) to reduce pulmonary artery
pressure is associated with a lower rate of hospitalisation due to
respiratory infection.
pharmacological treatment of COPD in patients with HF
Oral theophylline and mucolytics may be used in patients with severe

COPD, but data on their use in patients with HF are limited. In a small
study of euvolaemic patients with HF and central sleep apnoea, a 5-day
course of theophylline reduced the number of apnoeic and hypopnoeic
episodes and was well tolerated.152 However, theophylline metabolism
may be reduced in patients with HF,153 potentially increasing the risks of
adverse effects of the medication including arrhythmia.
Similarly, home oxygen is often prescribed to patients with end-stage
COPD, and may improve outcomes for those with severe hypoxaemia.155–
157
In the only RCT of home oxygen therapy for patients with severe HF,
there was no difference in quality of life or measures of breathlessness
after 6 months of treatment in patients randomised to oxygen compared
to those receiving medical therapy alone
Inhaled β2-agonists are part of the mainstay of the COPD pharmacological

treatment. However, they can induce adverse cardiac effects in COPD
patients with pre-existing cardiovascular disease, especially in those
with coexistent HF. Patients with HF and concomitant COPD who require
a regular long-acting inhaled bronchodilator, should start treatment with
a long-acting antimuscarinic rather than long-acting β2-agonists
(entricular arrhythmias, vasoconstriction, angina pectoris, tachycardia, and palpitations; may
aggravate cardiomyopathy in patients with cardiovascular disease) These effects are due to several
processes: 1. β -mediated cardiac stimulation (no absolute β selectivity); 2. reflex tachycardia after β -

mediated vasodilation; and 3. the results of potential hypokalemia.
In fact, short course steroid treatment can induce a profound diuresis in

patients with HF and severe venous congestion who are not responding
to conventional diuretic therapy,150 and may be associated with better
outcomes amongst patients hospitalized with HF.

Tiotropium and ipratropium inhalers are often used in combination with
long-acting beta-agonists in patients with COPD who remain breathless
despite frequent use of a short-acting treatment.56 Both tiotropium and
ipratropium are associated with improved symptoms and a reduced risk
of COPD exacerbation.134 However, both are also associated with an
increased risk of cardiovascular and all-cause mortality in meta-
analyses,18,135 possibly due to pro-ischaemic or pro-arrhythmic effects
(Figure 3).136,137
