RE: BADEJO IFEOLUWA SAMUIEL/MALE/25 YEARS

I trust this correspondence finds you in good health. I am writing to formally communicate the medical condition of Mr.
Badejo Ifeoluwa Samuel, a 25years old male and prospective National Youth Service Corps (NYSC) member, who
recently underwent a comprehensive medical evaluation at our esteemed medical facility.

At his recent clinical appointment, our medical team observed a relapse of his ulcer with an exacerbation of acid,
indicative of a chronic state. Subsequent endoscopy results further disclosed the development of an outlet obstruction.
Regrettably, Mr. Badejo`s health has recently deteriorated due to environmental triggers, manifesting in recurrent, life-
threatening ulcer nausea and vomiting attacks over the past few months.

Considering the gravity of his medical condition, and after meticulous consideration by our medical team, it is our
professional opinion that his participation in the mandatory one-year National Youth Service Corps will significantly
impact his health management. Consequently, we strongly advocate for her redeployment to Ogun State, where he can
access consistent medical management and adhere to the prescribed regular check-ups.

We kindly implore your esteemed office to facilitate Mr. Badejo's redeployment on compelling medical grounds, taking
into account his imperative need for proximity to both his relatives in Ogun State and our hospital. We firmly believe that
such preferential attention is paramount to pre-empt emergencies and mitigate unwanted complications.
