Complete Bed Bath


Equipment :
Procedure C NYC
1. Gather necessary equipment.
2. Wash your hands. Put on gloves.
3. Identify yourself by name. Identify the resident by name. Explain procedure to the
resident. Speak clearly, slowly, and directly. Maintain face-to-face contact whenever
possible.
4. Provide privacy.
5. Offer bedpan/urinal then empty, clean, and put it away.
6. Remove gloves and discard in appropriate container. Wash hands. Put
on clean gloves.
7. Place client in supine position near the side of the bed nearest you.
8. Un-tuck bed linens.
9. Remove bedspread and blanket; fold and place on chair if reusing;
otherwise, place in laundry basket
10. Cover top sheet with a large towel. Ask the client to hold the towel in
place; if unable, tuck under client's shoulders.
11. Remove top sheet without disturbing the towel and place in laundry
basket at bedside.
12. Remove client's gown or pajamas.
13. Fill bath basin 2/3 full of warm water (115ºF). Check temperature with
inner aspect of arm.
14. Place a towel across the client's chest.
15. Wet washcloth and squeeze out excess water. Make a washcloth mitt.
16. Wash eyes first. Start at inner corner and work out. Use different area of
washcloth for each eye. Don't dangle the ends of the washcloth.
17. Wash, rinse, and dry face, ears, nose, and mouth.
18. Wash, rinse, and dry neck.
19. Expose arm farther from you; place towel under arm up to axilla.
20. If the client is able, place a basin of water on the bed and immerse
client's hand in water and wash.
21. Wash and rinse far shoulder, axilla, arm, and hand.
22. Remove the basin and dry the client's arm, shoulder, and hand.
23. Repeat steps 21-23 with arm closer to you.
24. The In-Home Aide may perform fingernail care at this time.
25. Place towel across chest.
26. Wash and rinse chest and breasts while lifting towel.
27. Dry skin thoroughly.
28. Keep chest covered with towel.

29. Wash, rinse, and dry abdomen.
30. Change bath water in basin. Obtain a clean washcloth.
31. Expose the farther leg; flex (bend) leg and place bath towel lengthwise
under the leg up to the buttocks
32. Wash and rinse leg and foot.
33. Dry leg, foot, and in between toes.
34. Repeat steps 32-34 on leg nearer you; cover client with bath blanket.
35. May perform toenail care at this time.
36. Place the towel and washcloth in a laundry basket and get clean ones.
37. Change bath water in basin. Obtain a clean washcloth.
38. Ask or assist the client to turn on his side with back towards you.
39. Fold a towel over the client's side to expose his back and buttocks;
place clean towel parallel to client's back
40. Wash, rinse, and dry the client's back and buttocks.
41. Give backrub using warmed lotion.
42. Turn client to back; place clean towel under buttocks.
43. If client is able, provide wash cloth, soap, and towel and instruct him to
wash and dry peri area.
44. If the client is unable, wash peri area from front to back.
45. Place dirty linen in appropriate container.
46. Remove and dispose of gloves. Wash hands.
47. If client did own peri care, provide fresh water for client to wash hands.
48. Apply warmed lotion and deodorant as needed.
49. Put clean clothing on client without exposing him.
50. Remove, clean, and store equipment.
51. Wash your hands.
52. Make the client comfortable.
53. Record observations and report anything unusual to nurse/supervisor.

Making an Occupied Bed


Equipment : clean linen: mattress pad, fitted or flat bottom sheet, waterproof bed
protector if needed, blanket(s), cotton draw sheet, flat top sheet, pillowcase(s), gloves
Procedure C NYC
1. Gather necessary equipment.
2. Wash your hands. Put on gloves.
3. Identify yourself by name. Identify the resident by name.
4. Explain procedure to the resident. Speak clearly, slowly, and directly.

Maintain face-to-face contact whenever possible.
5. Provide for the resident’s privacy with curtain, screen, or door.
6. Place clean linen on clean surface within reach (e.g., bedside stand,
over bed table, or chair).
7. Adjust bed to a safe working level, usually waist high. Lock bed
wheels. Lower head of bed.
8. Loosen top linen from the end of the bed or working side. Cover the
resident with a cotton bath blanket or the loosened top sheet on the bed.
9. Raise side rail on far side of bed. After raising side rail, help resident to
turn onto side, moving away from you toward raised side rail.
10. Loosen bottom soiled linen on working side.
11. Roll bottom soiled linen toward resident. Tuck it in snugly against
resident’s back.
12. Place and tuck in clean bottom linen. Finish with bottom sheet free of
wrinkles. If you are using a flat bottom sheet, leave enough overlap on
each end to tuck under the mattress. If the sheet is only long enough to
tuck in at one end, tuck it in securely at the top of the bed. Make
hospital corners to keep bottom sheet wrinkle-free .
13. Smooth the bottom sheet out toward the resident. Be sure there are no
wrinkles in the mattress pad. Roll the extra material toward the resident. Tuck
it under the resident’s body.
14. If using a waterproof pad, unfold it and center it on the bed. Tuck the
side near you under the mattress. Smooth it out toward the resident.
Tuck as you did with the sheet.
15. If using a draw sheet, place it on the bed. Tuck in on your side, smooth,
and tuck as you did with the other bedding.
16. Help resident to turn onto clean bottom sheet. Raise side rail nearest
you.
17. Move to other side of bed and lower side rail.
18. Turn resident away from you toward side rail
19. Loosen soiled linen. Look for personal items. Roll linen from head to
foot of bed. Avoid contact with your skin or clothes. Place it in a
hamper/bag, at foot of the bed, or in a chair.
20. Pull and tuck in clean bottom linen just like the other side. Finish with
bottom sheet free of wrinkles.
21. Place resident on his back. Keep resident covered and comfortable,
with a pillow under the head. Raise side rail.
22. Unfold the top sheet. Place it over the resident and center it. Ask the
resident to hold the top sheet. Slip the blanket or old sheet out from
underneath. Put in the hamper/bag.
23. Place a blanket over the top sheet. Tuck the bottom edges of top sheet
and blanket under the bottom of the mattress. Make hospital corners on
each side. Loosen the top linens over the resident’s feet. This prevents
pressure on the feet. At the top of the bed, fold the top sheet over the
blanket about six inches.

24. Remove the pillow. Do not hold it near your face. Remove the soiled
pillowcase by turning it inside out. Place it in the hamper/bag.
25. With one hand, grasp the clean pillowcase at the closed end. Turn it
inside out over your arm. Next, using the same hand that has the
pillowcase over it, grasp one narrow edge of the pillow. Pull the
pillowcase over it with your free hand. Do the same for any other
pillows. Place them under resident’s head with open end away from the
door.
26. Make resident comfortable. Make sure sheets are free from wrinkles
and the bed free from crumbs.
27. Return bed to appropriate position. Remove privacy measures.
28. Dispose of soiled linen in the proper container.
29. Remove gloves. Wash your hands.
30. Document procedure using facility guidelines.

Making an Unoccupied Bed


Equipment : clean linen: mattress pad, fitted or flat bottom sheet, waterproof bed
protector if needed, blanket(s), cotton draw sheet, flat top sheet, pillowcase(s), gloves
Procedure C NYC
1. Gather necessary equipment.
2. Wash your hands. Put on gloves.
3. Place clean linen on clean surface within reach (e.g., bedside stand,
over bed table, or chair).
4. Adjust bed to a safe working level, usually waist high. Lock bed
wheels. Put bed in flattest position.
5. Loosen soiled linen. Roll soiled linen (soiled side inside) from head to
foot of bed. Avoid contact with your skin or clothes. Place linen in a
hamper/bag, at foot of the bed, or in chair.
6. Remove gloves. Wash your hands. Put on gloves.
7. Remake the bed. Spread mattress pad and bottom sheet, tucking under.
Make hospital corners to keep bottom sheet wrinkle-free. Put on
mattress protector and draw sheet. Smooth and tuck under sides of bed.
8. Place top sheet and blanket over bed. Center these, tuck under end of
bed and make hospital corners. Fold down the top sheet over the

blanket about six inches. Fold both top sheet and blanket down so
resident can easily get into bed. If resident will not be returning to bed
immediately, leave bedding up.
9. Remove the pillow. Do not hold it near your face. Remove the soiled
pillowcase by turning it inside out. Place it in the hamper/bag. With
one hand, grasp the clean pillowcase at the closed end. Turn it inside
out over your arm. Next, using the same hand that has the pillowcase

Making a Surgical Bed


Equipment : clean linen: mattress pad, fitted or flat bottom sheet, waterproof bed
protector if needed, blanket(s), cotton draw sheet, flat top sheet, pillowcase(s), gloves

2. Wash your hands. Put on gloves.
3. Place clean linen on clean surface within reach (e.g., bedside stand,
over bed table, or chair).
4. Adjust bed to a safe working level, usually waist high. Lock bed
wheels. Put bed in flattest position.
5. Loosen soiled linen. Roll soiled linen (soiled side inside) from head to
foot of bed. Avoid contact with your skin or clothes. Place linen in a
hamper/bag, at foot of the bed, or in chair.
6. Remove gloves. Wash your hands. Put on gloves.
7. Make an unoccupied bed with bedding left up (a closed bed). Do not
tuck top linens under the mattress.
8. Fold top linens down from the head of the bed and up from foot of the
bed.
9. Fanfold linens lengthwise to the side away from door. Fanfolded
means folded several times into pleats.
10. Put on clean pillowcases. Place the pillows on a clean surface off the
bed (e.g., bedside stand, overbed table, or chair).
11. Leave bed in its highest position. Leave both side rails down.
12. Move all furniture to make room for the stretcher.
13. Dispose of soiled linen in the proper container.

Helping a Client to Sit-up


Procedure C NYC
1. Wash your hands.
2. Identify yourself by name. Identify the resident by name.
3. Explain procedure to the resident. Speak clearly, slowly, and directly.
Maintain face-to-face contact whenever possible.
4. Ask any visitor to step out of the room, if appropriate
5. Roll the client on his side, facing you. Bend his knees
6. Reach one arm over to hold him in back of his knees
7. Place your other arm under the neck and shoulder area
8. Position your feet with a wide base of support and your center of
gravity close to the bed
9. On the count of three, shift your weight to your back leg. While you
are doing this, swing the client’s legs over the edge of the bed while
pulling his shoulders to a sitting position
10. Remain in front of the client with both hands on him for support. Do
not leave him until you are sure he is stable
11. Proceed with the remainder of the transfer. (for a client who requires
only little assistance, the procedure remains the same. Direct the client
through the steps above and support him when necessary. Be sure to
remain with him in the sitting position until he is stable).

Helping a Client to Stand


Procedure C NYC
1. Wash your hands.
2. Identify yourself by name. Identify the resident by name.
3. Explain procedure to the resident. Speak clearly, slowly, and directly.
Maintain face-to-face contact whenever possible.
4. Ask any visitor to step out of the room, if appropriate
5. Move to the front of your chair or bed. Put your hands on the arms of
the chair. The client is sitting over the edge of the chair or bed.
6. Place one of your knees between his knees. Your feet should be in
good position and you should be close to the chair or bed. If the client
has a weak knee, brace it with your knee.
7. Put one foot in under you. This should be the strongest leg. Bend your
knees and lean onto your forward foot to place the same side arm
around the client’s waist. You have now encircle \d the client and are
holding him at his center of gravity.
8. On the count of three, push down with your arms, lean forward, stand
up. Remember to count to three. It allows you both to know when to
start the motion and work as a team. Hold the client closely. The more
assistance needed, the closer you hold the client. On the count of three,
rock your weight to your back foot.
9. Remain in front of the client with both hands on him for support. Do
not leave him until you are sure he is stable
10. Proceed with the remainder of the transfer. (for a client who requires
only little assistance, the procedure remains the same. Direct the client
through the steps above and support him when necessary. Be sure to
remain with him in the sitting position until he is stable).

Pivot Transfer from Bed to Chair


Equipment : Wheel chair; Gait belt/ Transfer belt
Procedure C NYC
1. Gather necessary equipment.
2. Place the wheel chair at a 45º angle to the bed. Place the chair so that
the client will move toward his stronger side.
3. If you are transferring the client to a wheel chair, lock the wheels of the
chair. If the bed has wheels, lock these wheels, too.
4. Wash your hands. (Put on gloves).
5. Identify yourself by name. Identify the resident by name.
6. Tell the client what you are going to do.
7. Bring the client to a sitting position with his legs over the edge of the
bed.
8. Explain the procedure to the client:
9. When you are sure that the client understands the procedure, perform
the transfer.
10. Secure the client in the chair. Make him comfortable. Leave him in a
safe place.
11. Wash your hands.
12. Chart any observations you may have made during this procedure.

Pivot Transfer from Chair to Bed


Equipment : Bed; Gait belt/ Transfer belt
Procedure C NYC
1. Wash your hands.
2. Prepare the bed.
3. Place the chair at 45º angle to the bed so the client moves towards his
stronger side.
4. If you are transferring the client from a wheelchair, lock the wheels of
the chair. If the bed has wheels, lock these too.
5. Identify yourself by name. Identify the resident by name.
6. Tell the client what you are going to do.
7. Get into a good position using a firm base of support and proper body
mechanics.
8. Direct the client to come to a standing position.
9. Direc t the client to reach for the bed and pivot. Help and guide him.
10. Make the client comfortable. Reposition the side rails.
11. Wash your hands.
12. Chart any observations you may have made during this procedure.

Sterilizing Bottles
Equipment : Bottles; nipples; caps; jar; bottle brush; dish detergent; hot water from the
tap; large pot with cover or a special sterilizing pot for baby bottles; small towel; tap
water; stove / heat source for cooking, timer, tongs.
Procedure C NYC
1. Gather necessary equipment.
2. Wash your hands.
3. Scrub bottles, nipples and caps with hot soapy water. Use the bottle brush to
clean inside the bottles. Always squirt hot soapy water through the holes in
the nipples to clean out any dried-on formula.
4. Rinse thoroughly with hot water.
5. Fold the small towel to fit in the bottom of the pot, and lay it there. This will
prevent the bottles from breaking. (this is done when you do not have a bottle
rack)
6. Stand the washed bottles on the towel in a circle around the inside of
the pot.
7. Place the caps and nipples into the clean, empty jar. Place into the pot
at the center of the bottles.
8. Pour water into and around the bottles and into the jar with the nipples
until 2/3 of each bottle is under water.
9. Cover the pot.
10. Place the pot on the stove burner and turn on the burner to the high or
full setting.
11. When the water comes to a full boil, begin timing. Allow the water to
remain at a full boil for 25 minutes.
12. Remove the jar with the nipples and caps 10 – 15 minutes after the full
boil begins. With the nipples still inside the jar, stand the jar on the
table to cool.
13. Turn off the burner.
14. Take the cover off the pot and allow it to cool.
15. Remove the sterile bottles from the pot with sterile tongs.
16. Wash your hands.

Giving an Infant a Tub Bath


Equipment : infant tub or sink; two bath towel soft; cotton balls; washcloth; warm

water; baby soap; baby shampoo; baby powder; lotion or cream; diaper; clean clothes.
Procedure C NYC
1. Gather necessary equipment.
2. Wash your hands.
3. Wash the sink or tub with a disinfectant cleanser and rinse thoroughly.
4. Line the sink or tub with a bath towel.
5. Place a towel on the counter next to the sink or tub, as you may want to
lay the infant down to dry him.
6. Fill the tub or sink with 1-2 inches of warm water (warm to the touch
of the elbow)
7. Undress the infant, wrap him in a towel or blanket and bring him to the
tub or sink.
8. Using a cotton ball moistened with warm water and squeezed out,
gently wipe the infant’s eyes from the nose toward the ears. Use a
clean cotton ball for each eye.
9. To wash the hair, hold the infant in the football hold, with the baby’s
head over the sink or tub. This will free your other arm to wet the hair,
apply a small amount of shampoo, and rinse the hair.
10. Dry the infant’s head with a towel.
11. Unwrap the infant, and gently place him on the towel in the sink or tub.
One of your hands should always be holding the baby. Never let go,
not even for a second.
12. Wash the infant’s body with the soap and the washcloth, being careful
to wash between folds (creases) of the skin.
13. If the infant is female, always wash the perineal area from front to back.
14. If the infant is male, clean the foreskin by gently retracting it. If the
child has been circumcised, this is not necessary.
15. Rinse the infant thoroughly with warm water.
16. Lift the infant out of the water and onto the towel you laid out on the
counter.
17. Dry the infant well, being careful to dry between folds of the skin.
18. Now you can apply powder, lotion or cream to the infant, whichever
the mother prefers or as instructed by your supervisor.
19. Diaper and dress the infant.
20. Place the infant in his crib or allow the mother to hold him. Show the
mother how to hold the infant in either the upright or the cradle
position.
21. Clean and return the equipment and supplies to their proper place.
22. Clean the area where the bath was given.
23. Wash your hands.
