    SPECIAL POWER OF ATTORNEY

    I, JOHN VINCENT REYES DE CASTRO, of legal age, residing at Sandoval Subd.,


    Brgy. Wakas Tayabas City, hereby grant a Special Power of Attorney to my mother,
    SEVERINA R. DE CASTRO, and/or my wife, MELISSA M. DE CASTRO, of legal
    ages, residing at Sandoval Subd., Brgy. Wakas, Tayabas City. collectively referred to as
    my "Attorneys-in-Fact," for the purpose of obtaining a copy of my Transcript of Records
    (TOR) from Southern Luzon State University Lucban and performing any necessary legal
    actions related to this matter.

    The specific powers granted to my Attorneys-in-Fact include, but are not limited to:

    1. Requesting and collecting my Transcript of Records on my behalf.


    2. Signing any necessary documents or forms required by the university.
    3. Representing me in all matters related to obtaining the TOR.
    4.The Attorneys-In-Fact will process any requirements for DFA - Apostille claim and sign
    necessary documents as needed.
    5. Any other actions necessary to fulfill the purpose of this Special Power of Attorney.

    I understand that this Special Power of Attorney is valid for the sole purpose mentioned
    above and does not grant my Attorneys-in-Fact the authority to act on my behalf in any
    other legal matters.

    This Special Power of Attorney shall be in _________ to _________, unless earlier
