Serum iron can be decreased in conditions like iron deficiency anemia and in inflammatory disorders (acute infection, immunization, and myocardial infarction),
Hemorrhage etc. Increased serum iron can be seen in conditions like hemochromatosis, hemolytic anemia, hepatitis, Iron poisoning and Frequent blood transfusions
Total Iron Binding Capacity (TIBC), Serum 416 ug/dL 250-400 Ferrene
Clinical significance:-
TIBC (total iron-binding capacity)—measures the total amount of iron that can be bound by proteins in the blood. Since transferrin is the primary iron-binding protein,
the TIBC test is a good indirect measurement of transferrin availability—the amount of transferrin that is available to bind to iron. TIBC is increased when the body's
iron supplies are low. This can occur in conditions like iron deficiency anemia, pregnancy (late) etc. Reduced TIBC may be seen in hemolytic anemia,
hypoproteinemia, Inflammation, cirrhosis of liver, Malnutrition, pernicious anemia, Sickle cell anemia.
Clinical significance:
Vitamin B12 (cobalamin) is necessary for hematopoiesis and normal neuronal function. The body uses its vitamin B12 stores very economically, reabsorbing vitamin B12 from the
ileum and returning it to the liver; very little is excreted. Vitamin B12 deficiency may be due to lack of IF secretion by gastric mucosa (eg, gastrectomy, gastric atrophy) or intestinal
malabsorption (eg, ileal resection, small intestinal diseases). Pernicious anemia is a macrocytic anemia caused by vitamin B12 deficiency that is due to a lack of IF secretion by gastric
mucosa. Serum methylmalonic acid and homocysteine levels are also elevated in vitamin B12 deficiency states.
Clinical Significance: -
