Fire Investigation Report


Date : 09.05.2024

Time : 6.30 AM

Location : SMS#1 ( Stores)

Brief Description of the fire:

At 6.30 AM a fire call was received by fire dept. from Mr. Rohan of SMS#1.
Immediately the fire crew responded to the call and reached the incident site
within 2 minutes. After reaching at the spot they found that the mechanical stores
which is located below the CCM is on fire. So, Immediately they started expelling
water on the fire by fire tender to control the propagation of fire. Within 30
minutes the fire was brought under control and within 45 minutes the fire was
completely extinguished.

After the investigation it was revealed that the hot Molten metal spatter got
splashed from the ladle during the time of shifting from furnace to CCM and
entered the store room resulting in ignition of the combustible material. It was a
Class”A” Fire so could be quenched by water as media of extinguishment. After
interrogation with Mr. Rohan ( Stores Incharge) it was evident that the store room
was used for storing scrap material such as electrical cable , Hose pipes and
spares of furnace. Also Mr. Sanjay Kumar Yadav was contacted to know the root
cause of the fire and he said that it might be from the spatter of hot molten
matter.

Recommendation:

No combustible or flammable material should be stored in and around the


furnace as well as CCM where the spatters from the hot metal may cause fire .
The stores should be made up of concrete wall and should not have any openings.
It should be well ventilated and well lit and should have one emergency exit. No
inflammable material should kept inside the store. Ensure availability of fire
extinguisher at the entrance of the stores. Good house keeping should be
maintained inside the stores. Hot spatter should be contained inside the ladle by
applying some engineering control. While shifting the hot metal with ladle the
shift in charge should ensure that the ladle is filled upto 3/4 th of its volume as per
Factories act 1948. The crane operator should ensure that there should not be
any jerk or lateral movement of the ladle to prevent the spillage of hot metal. All
the electrical wiring should be concealed inside the walls to prevent any open
sparks. No smoking zone to be displayed at the entrance door.

Conclusion: There was no such damage to life except for property damage which
is estimated to be of Rs 6000 /- .
